Comparison study of total vaginal hysterectomy and total single port laparoscopic hysterectomy for uterine benign diseases

목적: to compare the surgical outcomes and the effects on health realted quality ( HRQOL) of total vaginal and single port laparoscopic hysterectomy . 방법: Among 121 patients who underwent hysterectomy for benign uterine lesions, 58 patients underwent total vaginal hysterectomy. 63patients underwent total single port laparoscopic hysterectomy. We analyzed the surgical and clinical parameters for the all patients. HRQOL data from the 36-item short form(SF-36) questionnaire were recorded in all patients. Surgical outcoms, complications, and HRQOL were compared between patients in two hysterectomy groups. 결과: There were no statistically significant differences in the age, uterine disease type, body mass index, blood loss, However, the number of previous operation( p=0.4 ) combined operation (p<0.01 )and complication rates (p<0.01 ) were increased in single port laparoscopic hysterectomy group. Mean operative time(p=0.03 ) was increased in vaginal hysterectomy group. All SF-36scales did not shows significant differences between two groups. For each groups, there were no significant changes of preoperative and postoperative HRQOL. 결론: Compared with total vaginal hysterectomy Total single port laparoscopic hysterectomy was preferred for the patients who had more previous operations and need other gynecologic intraabdominal operations. While Total single port laparoscopic hysterectomy can cause more complications and can not improve HRQOL.
