The mainstay treatment of extramammary Paget’s disease (EMPD) is surgical excision. But surgery consistently accompanies the risk of compromising genitourinary functions. Nonsurgical treatments including radiotherapy (RTx) and photodynamic therapy (PDT) are considered in such cases. We report a case of EMPD patient who showed resistance to PDT and surgery but was cured through radiotherapy. The patient was a 68-year-old man confirmed as EMPD histologically and was primarily treated with PDT, total of 17 sessions and 2 surgeries. Despite treatment, the lesion persisted and he received RTx with a total dose of 6700 cGy. His follow up biopsy did not show any Paget cells. PDT has been considered a potentially useful treatment modality in EMPD due to its ease of accessibility and lack of long-term complications. However, as in our case, RTx might significantly resolve EMPD lesions resistant to PDT.
