2009년 OECD통계에 따르면 사회복지지출, 보건지출 등은 하위권에 위치해 있다. 이는 경제발전과 성장에 치중한 정책이 성공한 반면 분배에 관하여 소홀 했다는 것을 보여준다. 고령사회에 진입한 우리나라는 일반적인 노인문제의 요인으로 꼽는 고령화와, 핵가족화, 그리고 출산율에 있어서 OECD국가 중 가장 심각한 증가율을 보이고 있다. 정부는 이러한 사회현상을 해결하기 위하여 2008년 7월 노인장기요양보험을 시행함으로서 본격적으로 고령사회의 문제를 정책적으로 대비하고 있다. 비용에 관한 정책적 문제 이외에 다른 요인에 관하여서는 급성병상과 요양병상, 너싱홈, Daycare center 등 시설의 등급에 따른 시설복지의 관리가 종합적으로 이루어져야 하며, 상대적으로 사회적비용이 적게 드는 재가복지의 균형적 분배를 해결하는 것이 급선무이다. 한정된 복지비용에서 최대한의 효율적 서비스를 제공 할 수 있는 방법으로 시설 계획적 측면에서 병상관리와 시설운영의 측면에서 통합적 케어를 개발하는 것이다. 따라서 본 연구에서는 우리나라의 의료와 복지의 이원화되어 있는 전달체계를 종합적으로 고찰하고, 먼저 노인문제를 접한 다른 나라들의 대처방법 중 통합적 케어의 방법론을 국내에 적용 할 수 있는 노인의료복지 클러스터를 제안하여, 노인의료복지시설 계획에 필요한 기초적 자료를 제공하는데 연구의 목적이 있다.

In July 2008, To resolve this social phenomena, The government is prepared the conducting elderly long-term care insurance for the problem of aging society. In addition to policy issues concerning the cost, other factors should be done synthetically the management of facilities welfare rating based on and social cost-effective household welfare`s balanced distribution should be resolved relatively. The way of cost-effective welfare services in the most limited welfare costs is to develop the bed management in terms of well-planned facility and the Integrated care in terms of facility management. Therefore, this study is to consider synthetically on delivery system of the duality of the treatment and welfare of the country. first of all, the methods of integrated care to deal with other country faced the problem of the elderly can be applied to domestic elderly care and proposed welfare cluster, aims to provide basic data for planning research purposes.
