재정영향분석(Budget Impact Analysis, BIA)은 선별등재목록에 의약품을 등재함으로써 지불자의 (의약품) 예산에 미치는 재정적 영향을 보기 위한 것이다. 분석에 필요한 자료는 광범위 하고, 질병의 발생률과 유병률 자료를 포함하고, 기존 치료법이 어느 정도 대체될 것인가, 치료 기간, 의약품 가격, 시장 점유율 기대치와 총 비용 기대치 등을 포함한다. 우리나라는 2007년부터 경제성평가 요소가 의약품 상환 및 가격결정에 영향을 미치는 주요 요소가 되었으나, 보험재정의 지불가능성을 해결해 줄 수 있는 재정영향분석에 관한 포괄적인 지침은 별도로 마련되어 있지 않은 상태이다. 이 연구는 재정영향분석 가이드라인 개발의 필요성 및 개발항목 등을 살펴보기 위해 전문가 의견조사를 실시하였다. 총 8명의 전문가가 참여한 조사결과, 별도의 재정영향분석 가이드라인 개발 필요성이 인식되고 있었으며, 중요도와 실행가능성 점수가 높은 항목을 고려하여 가이드라인 구성항목을 개발하는 것이 바람직하다고 제안하고 있다.

Budget Impact Analysis(BIA) is a tool to find out financial impact on positive list on purchaser of pharmaceuticals according to listing new drugs. Data includes various range of disease incidence rate, prevalence rate, substitution rate, time period, drug price, market share and total cost estimate, etc. In Korea, cost-effectiveness of new drug becomes as a decision criteria for reimbursement and price negotiation in decision process since 2007. Although it includes some data requirements for BIA, but it is not comprehensive. This study carried survey of experts` opinion to find out the necessity of development BIA guidelines fit to Korean specific situation, and importance and feasibility of data requirements for BIA guideline.
