We report a case of a patient with acute pelvic pain. The patient showed a voluminous mass situated in the upper abdomen and contiguous to the uterus by imaging scan. Laparotomic surgery confirmed the diagnosis of uterine subserosal leiomyoma resulting caudal stretch and axial torsion of lower body of uterus and cervix.
