목적 : 심한 급성망막괴사 환자에서 망막박리가 발생되기 전에 수술적 치료를 시행한 후 결과를 알아보고자 한다. 증례요약 : 심한 급성망막괴사증으로 진단받은 2명의 환자를 초기에 항바이러스 제재, 부신피질 호르몬, 항응고제로 치료하였으며, 망막박리를 예방할 목적으로 유리체절제술 및 실리콘기름 주입술을 시행하였다. 2안 모두 경과 기간내에 망막박리가 발생되지 않았다. 결론 : 급성망막괴사 환자 대부분에서 발생되는 망막박리는 불량한 시력예후를 갖게하는 중요한 원인이 된다. 이러한 망막박리에 대한 예방적 치료로서 유리체절제술 및 실리콘 기름 주입술을 시행하는 것은 효과적이라고 생각하는 바이다. <한안지 49(3):519-524, 2008>

Purpose: We report the results in two patients received prophylactic surgery to prevent retinal detachment in the severe acute retinal necrosis. Case summary: Two patients in whom the presumptive diagnosis of the severe acute retinal necrosis were concomitantly treated with antiviral agents, systemic steroids and anti platelet therapy. We performed vitrectomy and silicone oil tamponade as the prophylactic surgical procedure for retinal detachments. The retinas remained attached in all patients over a follow up periods. Conclusions: Retinal detachment secondary to acute retinal necrosis has a complex nature and usually results in severe visual loss. A combination of vitrectomy and silicone oil tamponade may be effective prophylaxes against retinal detachment in acute retinal necrosis. J Korean Ophthalmol Soc 49(3):519-524, 2008
