목적: 유리체절제술시 내경계막 제거술을 시행한 후 광간섭단층촬영(OCT)에서 이상을 보인 경우를 분석하고자 하였다. 대상과 방법: 본원 안과에서 한명의 술자에 의해 유리체 절제술과 병행하여 내경계막 제거를 시행받은 22명 22안의 의무기록을 후향적으로 조사하였다. 환자의 나이, 성별, 안저검사소견, 최대교정시력, 수술방법, 합병증, 술 후 OCT 결과를 조사하였다. 결과: 수술 전 진단명은 망막전막 12안, 황반원공 5안, 미만성 당뇨황반부종 5안이었다. 내경계막 제거시 18안은 ICG를, 1안은 트리암시놀론을 이용하였고, 3안에서는 염색을 하지 않았다. 수술 후 교정시력은 망막전막, 미만성 당뇨황반부종에서는 유의한 호전을 보였고(p<0.05) 황반원공에서는 유의한 변화가 없었다(p>0.05). 술 후 OCT 소견으로는 17안에서 중심와 옴폭패임과 두께가 정상으로 회복되었으나 내경계막 제거시 ICG를 이용한 환자중 3안에서는 불규칙한 황반부 얇아짐이 관찰되고, 1안에서는 황반부의 낭포성 변화가 관찰되었다. 트리암시놀론을 사용하였던 1안에서는 반복적인 낭포황반부종이 발생하였다. 수술 중 합병증은 없었고 술 후 황반원공은 모두 예쇄되었으며 망막전막의 재발은 발견되지 않았다. 결론: 내경계막 제거술 후 황반부 조직의 변형은 술 후 최종시력에 통계적으로 유의한 영향을 미치지는 않았지만, 황반부 미세 구조에 변화를 가져올 수 있으므로 수술시 주의를 요하며 수술 후 세심한 추적관찰이 필요하다 <한안지 49(2):253-260, 2008>

Purpose: To analyse the postoperative anatomical changes of maculae using optical coherent tomography after internal limiting membrane peeling during vitrectomy. Methods: A retrospective review of 22 patients (22 eyes) who underwent vitrectomy with internal limiting membrane peeling was performed. Age, sex, fundus findings, BCVA, operation methods, complications, and postoperative OCT findings were recorded. Results: Of 22 eyes, 12 eyes had epiretinal membranes, 5 eyes had macular holes, and 5 eyes had diffuse diabetic macular edemas. Eighteen of 22 eyes underwent internal limiting membrane peeling using ICG, with 1 eye treated with triamcinolone and 3 eyes treated with nothing. The postoperative improvement of BCVA was statistically significant in epiretinal membrane and diffuse macular edema patients (p<0.05), but insignificant in macular hole patients (p>0.05). Postoperative OCT findings of 17 eyes showed that they had returned to normal shape and thickness. However, among the 18 eyes that underwent ICG dye-assisted ILM, 3 eyes showed irregular macular thinning and 1 eye showed macular cystic changes. One eye treated with triamcinolone presented with a recurrent cytoid macular edema. No postoperative complications or recurrences were reported by epiretinal membrane patients. Conclusions: Changes to macular shape did not significantly affect the final BCVA after internal limiting membrane peeling, but they can cause changes to the microstructure of maculae.
