목적: 비구 후벽의 분쇄 골절에 대하여 스프링 금속판을 이용하여 내고정한 후 임상적 방사선학적 결과 그리고 합병증에 대하여 분석하고자 하였다. 대상 및 방법: 2002년 3월부터 2004년11월까지 지연 나사나 하나의 금속판만으로 안정적 고정을 얻기 어려워 스프링 금속판을 이용하여 내고정한 10예의 비구 후벽을 침범한 골절을 대상으로 하였다. 술 후 정복의 정도를 Matta에 의한 분류와 컴퓨터 단층 촬영을 이용하여 분류하였으며 임상적 결과를 Matta의 기준을 이용하여 분석하였다. 결과: Matta의 분류에 따른 해부학적 정복이 6예, 만족할 만한 정복이 3예였으며, 불만족스러운 정복이 1예였다. 추시기간중 고정의 실패 없이 골유합을 얻었으며 임상적 결과는 우수가 4예, 양호가 4예, 보통이 2예였으며 불량은 없었다. 합병증으로는 스프링 금속판의 관절내 자극이 있어 조기 내고정물 제거를 시행하였다. 결론: 비구 후벽을 침범한 골절에 대하여 스프링 금속판을 이용하여 내고정한 결과 비록 단기 추시이지만 좋은 임상 결과를 얻어 지연 나사나 하나의 금속판 만으로 고정이 어려운 비구 후벽 골절 치료에 유용한 치료법으로 생각된다.

Purpose: This study was performed to evaluate the clinical results and complications of acetabular posterior wall fractures managed with spring plate technique. Materials and methods: We reviewed 10 cases of acetabular fracture involving the posterior wall managed with spring plate technique between March 2002 and November 2004. The radiologic results were evaluated by Matta`s method. Articular displacement was also evaluated in postoperative CT scan made after the operation. The clinical results were evaluated by Matta`s criteria. Results: According to the above criteria, anatomical reduction was obtained in 6 cases, satisfactory reduction in 3 and unsatisfactory reduction in 1. All cases were united without any loss of reduction. For the clinical results, 4 cases were classified as excellent, 4 as good and 2 as fair. As for complications, femoral head irritation due to the spike of spring plate was observed in one case. Conclusion: Fixation of the posterior wall fragment using spring plate was considered to be a useful method for posterior wall fracture with comminution involving articular margin.
