열공망막박리에 대한 공막돌륭술 후,수술 전 발견된 망막열공이 수술 후 잘 폐쇄되어 있음에도 불구하고 망막하액이 지연 흡수되는 증례들을 드물지 않게 관찰할 수 있다. 이러한 경우에 망막열공의 불완전 폐쇄 및 증식유리체망막병증 등과의 감별이 필요하다. 본 연구에서는 망막하액의 지연 흡수와 관련된 수술전후의 인자와 원인을 알아 보고자 하였다. 첫 망막수술로서 공막돌륭술을 시행받고 3개월이상 관찰이 가능하였던 원발 열공망막박리의 연속증례 84안(84명)을 대상으로, 수술 후 6일 이내에 망막이 완전 재유착된 군(조기 재유착군: 65안)과 7일 이상 망막하액이 다소간에 남아 있던 군(지연 흡수군: 19안)으로 분류하여 양쪽 군간의 임상적 특징을 분석하였다. 망막하액 지연 흡수군에서, 망막하액 흡수기간의 중간치는 58일(7일~2 2개월)이었다. 망막하액의 지연 흡수와 관련있는 술전 인자는 열공편의 존재, 증식유리체망막병증, 하측의 망막박리 및 고령이었다(p<0.05). 지연 흡수군에 동반된 안저 소견으로는 망막하 침착물, 망막하 띠모양 섬유증식, 분계선 등이 있었다. 이상의 결과는 망막하액 지연 흡수의 원인으로서 망막색소상피세포의 기능저하와 유리체로부터의 견인력이 복합적으로 작용하고 있음을 시사한다.(한안지 41:1177~1186, 2000).

Delayed absorption of subretinal fluid(SRF) occurs not so infrequently after scleral buckling for rhegmatogenous retinal detachment and needs to be differenciated from the condition caused by incomplete closure of retinal break or by proliferative vitreoretinopathy. The purpose of this study was to identify perioperative factors associated with delayed absorption of SRF after scleral buckling. Eighty-four consecutive cases of rhegmatogenous retinal detachment had undergone scleral buckling and been followed for more than 3 months. The duration was shorter than 7 days in early reattachment group(group A, n=65) and the duration was the same as 7 days or longer in delayed absorption group(group B, n=19). Perioperative demographic and ocular factors were analysed. Old age, inferior retinal detachment, operculated tear and proliferative vitreoretinopathy were more prevalent in group B than in group A(p<0.05). Subretinal gliotic band, demarcation line and subretinal precipitate were detected in group B. Our results suggest that the hypofunction of retinal pigment epithelium and the vitreous traction may play a role in delayed absorption of SRF(J Korean Ophthalmol Soc 41:1177~1186, 2000).
