백내장과 녹내장이 병발한 경우, mitomycin C(0.2mg/ml)를 사용하여 병용수술을 시행한 29안(21명)을 대상으로 안압과 시력에 대한 결과와 합병증을 알아보았다. 평균 추적관찰은 6.8+-1.8개월이었다. 수술전 평균안압은 26.3+-9.1mmHg(range,11-44mmHg)였으며, 수술후 마지막 추적관찰시의 안압은 14.2+-3.4mmHg(range,7-22mmHg)이며 12.1mmHg만큼 하강하였다. 마지막 내원시 21안(72.4%)에서는 안압이 21mmHg이하이고 안압조절약제가 필요하지않아, 완전한 성공(complete success)으로 보았다. 안압조절에 필요한 약제는 수술전에 평균 2.4+-1.0개(range,1-4개)였고, 마지막 내원시에 평균 0.4+-0.7개(range,0-2개)였다.모든 환자에서 안압조절을 위한 약재의 수가 수술전에 비하여 마지막 내원시 감소하였다. 수술후 마지막 내원시의 시력은 22안(75.9%)에서 0.5이상이었으며, 모든예에서 수술전보다 증진되었다. 수술후 발생한 합병증에는 전방출혈, 창상누출,낭포황반부종 등이 있었으며, fibrinous uveitis가 6예(20.7%)로 가장 많았다. 백내장과 녹내장 병용수술에서 저농도의 mitomycin C의 사용은 시력개선과 안압하강에 효과적이고 비교적 안전하였다.(한안지 37:1884~1892, 1996)

To evaluated the results for intraocular pressure(IOP), visual acuity and postoperative complications, we conducted a retrospective review of the recordis of 29 consecutive cases who had combined cataract and glaucoma surgery with mitomycin C(0.2mg/ml) in patients with coexisting cataract and glaucoma. The mean follow-up period was 6.8+- 1.8 months. The mean preoperative IOP was 26.3+-9.1mmHg(range, 11-44 mmHg), which decreasesd by mean 12.1mmHg to a mean postoperative IOP of 14.2+-3.4mm Hg(range, 7-22mmHg) as measured at the last follow-up. At the last follow-up, 21 eyes(72.4%) had IOPs of 21mmHg or less without glaucoma medications as complete sucess. The mean preoperative number of antiglaucoma mediciaton was 2.4+-1.0(ragne,1-4), which decreased to a mean number of antiglaucoma medication of 0.4+-0.7(range,0-2) at the last follow-up. All patients were using fewer medications at last follow-up than preoperatively. Twenty two eyes(75.9%) had a best-corrected visual acuity of 0.5 or better at the last fol-low-up. Last follow-up best-corrected visual acuity was better than the preoper-ative best-corrected visual acuity for all patients. Six eyes(20.7%) had fibrinous uveitis as the most common postoperative complication, and other complications were hyphema, wound leak and cystoid macular edema etc. The glaucoma triple procedure with adjunctive mitomycin C(0.2mg/ml) appears to be a safe and effective surgical technique for treating patients with coexisting cataract and glaucoma(J Korean Ophthalmol Soc 37:1884~1892, 1996).
