A Clinical Study of Parotid Tumor

This is a report of clinical study of 26 cases with a diagnosis of parotid tumor who were treated using various surgical procedures at Department of Surgery, Seoul National University, Hospital over a period of 5 years from Jan.-1-1976 to Dec.-31-1980(Late 5 years). We compared this result with the previous report from Seoul National University Hospital which was a clinical study of 21 cases of parotid tumor who had been treated with surgical procedures at Department of Surgery, S.N.U.H. over a period of 5 years from Jan-1-1971 to Dec-31-1975(early 5 years). According to this comparison of total 47 cases, we concluded as follows. 1) The sexual incidence of parotid tumor was similar between early 5 years and Late 5 years as 1.6: 1 and 1.2: 1 in male predominant. If we consider malignant cases only. male predominance was significant as 3.5: 1. 2) The patient's chief complaints was a painless mass and the duration of illness was 7.3 years in early 5 years but 3.6 years in Late 5 years. 3) The mean size of mass was 3.5 cm in diameter and the majority of masses was located at infraauricular region. 4) As the report of histopathological findings of 26 cases, benign tumor included 17(65%) and malignant included 9(35%). The benign tumor contained mixed tumor(88%) and the malignant tumor contained malignant mucoepidermoid ca.(45%) as majority. So there was no difference in the ratio of benign to malignant and histopathological findings between the Early 5 years and Late 5 years. 5) The surgical procedures were performed with wide excision and superficial lobectomy for benign tumor, and with various method from wide excision to total lobectomy for malignant tumors. But the enucleation, which was performed occasionally during Early 5 years, was not performed during Late 5 years. 6) The post-operative complications were occurred in 24% and this is much decreased rate comparing with the Early 5 years. 7) In all 3 cases which pre, operative incisional biopsy were performed, post-operative facial nerve paralysis was occurred. So the pre-operative incisional biopsy may be the risk factor of post-operative complication as facial nerve paralysis. 8) The recurred cases were much decreased recently as comparing the 4% in the Late 5 years to 43% in the Early 5years. 9) There was no evidence of recurrence in 12 cases which was able to follow up during this study period.
