Blood-born metastatic involvement of the stomach is a rare entity and unusual causes of gastrointestinal hemorrhage. Exceptions to this rule are seen most frequently in melanoma, breast cancer and lung cancer. A 4g year old female patient was referred for evaluation of UGI bleeding. She was performed left total mastectomy for breast cancer 5 years 7 months ago. She complained of bone pain and whole body bone scan revealed multiple metastatic bone lesions. Radiotherapy and chemotherapy with Methotrexate, 5FU, Cytoxan and prednisolone were done under the impression of bone metastasis from breast cancer. Two weeks later she passed bloody vomitus and melena. Gastrofiberscopic findings revealed two small (1X1 cm sized) nodular lesions on proximal antrum and angle with central umbilicated ulceration, so-called Bull's eye configuration. Endoscopic gastric mucosal biopsy showed undifferentiated carcinoma with the absence of transition between stomach epithelium and carcinomatous cells, which was suggestive of gastric metastasis from breast cancer.
