Background: One of the serious long-term complications ot diabetes mellitus is diabetic foot lesion, which is a major cause of morbidity and mortality in diabetic patients. Diabetic persons have a 15 fold higher risk of lower extremity amputation than nondiahetic individuals. It is reported that the predisposing factors of the diabetic foot lesion are diabetic neuropathy, macrovascular disease and hyperglycemia. In this study, the blood glucose of patients with diabetic foot lesions are normalized by insulin pump, and clinical features of this study are compared with previously reported studies in which the blood glucose of patients are controlled by conventional insulin therapy. Methods: We studied 19 patients with diabetic foot lesions treated by insulin pump from January, 1993 to December, 1994 at Kon-Kuk University Hospital. According to therapuetic results, these patients were divided into medical and surgical consequence groups. And this consequence was compared with previously reported studies. Results: 1) There were no significant differences in age, sex, and duration of diabetes mellitus between this study and previously reported studies. 2) The most common site of diabetic foot lesions in this study were heels (100%), but those in Huh and in Lee were toes. (59.5%, 33.8% respectively) 3) A level of mean blood glucose of this patient was normalized in one week after the beginning of the insulin pump therapy and thereafter mean blood glucose level was maintained normal. However injccted insulin dose of this patient was maximum at first week and thereafter the insulin dose was gradually decreased. 4) The variability of daily blood glucose was reduced at second week of hospitalization. 5) In this study of diabetic foot patients whose blood glucose were normalized by insulin pump, the healing rate of diabetic foot by medical treatment only was higher than the one of previously reported studies in which the blood glucose of patients were controlled by conventional insulin therapy. Conclusion: It is concluded that tight control of blood glucose by insulin pump may improve prognosis of diabetic foot lesions.
