저자들은 1993년 7월부터 1995년 6월까지 부산대학교병원 산부인과에 입원하여 악성종양으로 진단받고 cisplatin을 근간으로 하는 복합항암요법을 받은 43명의 환자를 대상으로 ondansetron 단독투여군과 ondansetron, dexamethasone 복합투여군으로 나누어 항구토효과 및 부작용을 비교하여 다음과 같은 겨과를 얻을 수 있었다. 1. Cisplatin투여후 24시간 이내에 발생하는 급성구토의 조절효과는 완전조절이 복합투여군에서는 78%(18.23), 단독투여군에서는 60%(12/20)로 복합투여군에서 우수한 효과를 보였으나 통계적 유의성은 없었다. 지연성 구토의 경우는 복합투여군에서는 65%(15/23), 단독투여군에서는 55%(11/20)로 복합 투여에서 다소 우수한 효과를 보였으나 역시 통계적 유의성은 없었다. 2. 오심에 대한 효과적인 조절은 복합투여군에서 82%, 단독투여군에서는 75%로 복합투여군에서 다소 우수한 것으로 보였으나 통계적 유의성은 없었고(p>0.05), 100 mm 눈금자를 이용한 평가에서는 복합투여군에서의 오심의 정도가 덜한 것으로 나타났으며(치료 후 평균 19.4 vs 33.8), 구토발현까지 시간의 비교에서도 단독투여군에서 10시간 20분, 복합투여군에서 19시간 40분으로 의미있게 긴것으로 나타났다(p

Chemotherapy induced nausea and vomiting is a still significant problem in the care of patients receiving cisplatin based combination chemotherapy. Existing antiemetic regimens, such as high dose metoclopramide have demonstrated clinical efficacy. Recently the more effective regimens, such as ondansetron or ondansetron combined dexamethasone hve been reported. Ondansetron, a serotonin antagonist have improved the treatment of chemotherapy induced nausea and vomiting. But the antiemetiv effect of delayed emesis is questionable. We compared the effectiveness and side effects of two antiemetic regimens using ondansetron alone versus ondansetron combined with dexamethasone, in the prevention of emesis induced by cisplatin based combination chemotherapy. A total of 43 patients were enrolled and assessable for efficacy. Patients who received ondansetron plus dexamethasone had a higher complete antiemetic response rate(acute emesis: 78% vs 60%, p0.05). Headache(9%), diarrhea(4%) and andominal colic(4%) were the most common antiemetic related adverse events. The incidence of adverse events was similar between the treatment groups.
