저자들은 1992년 9월부터 1995년 8월까지 3년간 전남대학교병원 산부인과에 내원하여 초음파검사 및 진단복강경으로 자궁내막종으로 진단 받았던 65명의 환자를 대상으로, 그중 48명에서는 골반경하 자궁내막종 적출술을 받을 때까지 수술전 GnRH agonist치료를 시행하여 17명의 수술전 GnRH agonist 치료를 시행하지 않고 2~3개월후 수술을 시행한 대조군과 비교하여 수술전 GnRH agonist의 수술에 미치는 효과를 관찰하였다. 이 수술전 GnRH agonist의 효과를 판정하기 위하여 진단시와 수술시의 낭종크기의 변화와 유착정도 및 자궁내막증 spot의 변화를 관찰하였고, 수술시의 낭종 박리의 난이도를 관찰하여 다음과 같은 결과를 얻었다. 1. 자궁내막종의 크기 감소는 GnRH agonist를 수술전 1회 치료한 군은 8명중 8명 전부에서 변화가 없었고, 2회 치료군도 8명중 2명에서만 크기 감소가 있어 17명중 1명의 크기 감소가 있었던 대조군보다 통계적으로 유의한 낭종 크기 감소가 없었으나, 3회 사용한 군은 9명중 5명에서 감소가 있었고(p

From September 1992 to August 1995, at the Department of Obstetrics and Gynecology, Chonnam University Hospital, we performed preoperative GnRH agonist treatment(1-6 months) and pelvicopic surgery for 48 patients with endometrioma, and also performed pelviscopic surgery only 2~3 months after initial diagnostic laparoscopy for 17 cases. This retrospective study was conducted to evaluate the effect of preoperative GnRH agonist treatment on laparoscopic cystectomy for endometrioma by comparing changes in size of cyst, presence of endometrial spots and degree of adhesion and diffculty in stripping of cyst between GnRH agonist-treatment group and control group. The results were as follows. The size of cysts were decreased in none of 8 cases with 1 month-treatment and in 2 of 8 cases with 2 month-treatment. These data showed no significant difference from that of control group(decrease in size of cyst in 1 of 17 patients). However the sizes of the cysts were decreased in 5 of 9 patients with 3 month-treatment, 6 of 10 patients with 4 month-treatment and all 13 patients with 5 or 6 month-treatment.
