1994년 10월부터 1995년 1월까지 자궁경부암 환자 4명을 대상으로 저 침습성 수술인 복강경을 이용하여 질식광범위 자궁전적출술 및 골반임파절절제술을 처음 실시하였고 아래와 같은 예비성적을 얻었다. 1. 각각 2예에서 type 2와 type 3 광범위 자궁적출술 및 골반임파절절제술을 시행 할 수 있었다. 2. 평균 수술시간은 복강경으로 247.00 +or- 37.04분(범위 185-315분)이었고 질식수술시 54.00 +or- 18.28분(범위 34-105분)이었다. 3. 복강경 수술로 제거된 임파절수는 평균 18.25 +or- 4.47개(8-26)였고 전이된 임파절은 없었다. 4. 수술중 출혈은 복강경술식시 평균 237.50 +or- 88.58ml(50-450ml), 질식 수술시 평균 205.75 +or- 115.67 mm(70-523 ml)였고 혈관손상에 의한 대출혈은 없었다. 5. 평균 입원일수는 8.50 +or- 0.93 일(범위 6-10일)이었다. 6. 합병증은 2예로 각각 방광열상 및 수술후 방광기능 저하였고 합병증으로 개복수술 한 예는 없었다. 7. 수술시간, 임파절제거수, 입원일수는 다른 보고자와 비교시 유의한 차이가 없었다. 이상의 결과를 보면 복강경을 이용한 수술은 수술시간이나 임파절제거수나 입원일수나 출혈양에 있어서 저침습 수술의 장점 및 복강경수술이 축소수술이 아닌 광범위하게 암주위 조직을 적출 할 수 있고 자궁경부암 환자의 치료에 있어서 복강경을 이용한 수술방법은 단순의 진보된 테크닉이 아니고 독립된 수술의 한 방법으로서 다른 방향을 제시 할 수 있다고 사료된다. 앞으로 발달하는 복강경 수기로 수술이 훨씬 용이하게 될 것이며 효용성의 판정은 5년간 기다려 보아야 함으로 개복수술과 비교하여 어떤 수술방식이 환자에게 득이 될 수 있느지는 임상의들의 판단이 요한다고 사료되고 타 기관간의 비교연구 및 향후 조심스러운 장기 관찰이 필요하다고 사료된다.

I had performed laparoscopic assisted radical vaginal hysterectomy with pelvic lymphadenectomy in 4 cases of the cervical carcinoma(stage Ia2-IIa)during the period from Oct. 1994 to Jan. 1995. The scope of laparoscopic therapy could be extended to the cervical carcinoma without increased risk of morbidity. The following results were obtained 1. There were 2(Ia2) cases of laparoscopic assisted type 2 radical hysterectomy and 2( Ib, II a) cases of type 3 radical hysterectomy with pelvic lymphadenectomy. 2. Average operation time of laparoscopic phase and vaginal phase was 247.00 +or- 37.04 min(range 185-315 min) and 54.00 +or-18.28 min(range 34-105 min). 3. Average number of pelvic nodes obtained at laparoscopy were 18.25 +or- 4.47(range 8-26) and there were no metastatic lymph node. 4. Average blood loss at laparoscopic phase and vaginal phase was 237.50 +or- 88.58 min(range 50-450 ml) and 205.75 +or- 115.67 ml(range 70-523 ml). 5. Average stay in the hospital was 8.50 =or- 0.93 days (range 6-10 days). 6. There were 2 minor complication e.g., bladder laceration(1), bladder dysfunction(1). 7. There was no significant difference with other report(Querleu) about operation time and number of removed lymph node and hospital day( Wilcoxon 2-sample test).
