X , Y- 정자의 분리 및 운동성 변화에 관한 연구

1993년 4월 1일부터 1994년 12월 31일까지 가계에 유전질환이 존재하지 않고 생식기질환에 이환되지 않은 CASA에 의한 정액검사상 정상인 건강한 정자제공 자원자 30명을 대상으로 하여 90회를 정자 처리한 결과는 다음과 같았다. 1. 환자의 연령별 분포는 22세에서 32세로 평균연령은 25.5세 이었다. 2. Y -정자의 비율의 변화는 swim up , HSA, B-SA 방법에서 통계적으로 의미있게 증가하였 으며 (p<0.05), swim up, BSA의 경우 더욱 현저히 증가하였다. discontinuous percoll gradient , continuous percoll gradient 방법에서는 X-정자의 비율이 증가하였으며 (p<0.05) discontinuous percoll gradient 의 경우 더욱 현저히 증가하였다. sperm rise 방법에서는 Y-정 자의 비율이 약간 상승하였으나 통계적으로 의미가 없었다. (p<0.05). 3. 정자처리 후 운동성장자의 비율의 증가는 swim up, sperm rise, continuous percoll gradient ,discontinuous percoll gradient ,HSA, BSA의 6가지 처리방법 모두 통계적으로 유의하게 증가 하였다. (p<0.001). 4.정자지표 중 VSL,VCL경우는 swim up, sperm rise, continuous percoll, discontinuous percoll , HSA,BSA의 6가지 처리방법 모두 통계적으로 유의하게 증가하였으나 (P<0.05) LIN는 swim up, BSA 방법에서는 의미있게 감소하였고 (p<0.05) discontinuous percoll gradient , continuous percoll gradient 방법에서는 증가하였다. (p<0.05),ALH는 sperm rise, discontinuous percoll gradient 방법에서 증가하였다 (p<0.05). 5. 운동성정자의 회수율은 swim up 방법에서 21.6+-7.83%로 가장 낮았으며 discontinuous percoll 방법에서 66.84+-11.55%로 가장 높았다. Swim up, HSA, BSA의 경우 현저히 낮았으 며 discontinuous percoll , sperm rise의 경우 현저히 높았다. 이상의 결과로 정자처리법은 운동성장자의 분리에 효과적인 방법으로 자궁강내 수정시 유용 하게 사용 가능할 것으로 사료되며, 반성유전이 있는 가계에서 percoll gradient 방법은 선택 적으로 X-정자의 비율을 높여서 여아를 출산하는데 유용할 것으로 사료되며, 어떠한 이유로 인하여 남아를 원할 경우 swim up방법과 albumin gradient 방법이 추천된다.

There has been attempts to select the sex of a child prior to conception in humans. Couples with those X-linked disease that are undetectable until delivery makes need for sex selection to avoid an affected child. Centrifugation, microelectrophoresis and density gradient sedimentation methods have been tried to separate X and Y bearing sperm in humans. Some techniques such as albumin gradient method, precoll gradient density method and swim up method have been reported as a possible tool in making a change in sex ratio in vivo study. In this study. we tried to find out the relationship among sperm preparation method and change in X, Y-bearing sperm ratio, motility, sperm parameters and sperm recovery rate. Ther subjects were 30 patients who ejaculate three times every three days from April, 1993 to December , 1994 without genetic and venereal disease. The authors used swim up, continuous percoll gradient density, discontinuous precoll gradient density, human serum albumin, bovine serum albumin as sperm preparation method. The staining material was quiacrime mustard dihydrochloride (Sigma, USA) for identifying F-body. And we observed the change in X, Y-bearing sperm ratio, motility, sperm parameters and recovery rate after sperm preparation .the result were as follows y-bearing sperm was increased significantly in swim up, human serum albumin, boving serum albumin method (p<0.05). Y-bearing sperm was decreased significantly in continuous percoll gradient density, discontinuous percoll gradient density method (P<0.05) But sperm rise method did not significantly change the Y-bearing sperm though the ratio was slightly increased (p>0.05). The motile sperm ratio was increased significantly in swim up, sperm rise, continuous percoll gradient density, discontinuous percoll gradient density, human serum albumin and boving serum albumin after sperm preparation (p<0.001). Straight line velocity(VSL) and curvilinear velocity (VCL) of sperm parameters were increases significantly in all sperm preparation method(p<0.05), Linearity(LIN)was decreased significantly in swim up and bovine serum albumin method, but increased in discontinuous percoll gradient density and conutiuous percoll gradient density (p<0.05). Amplitude of lateral head displacement(ALH) was increased significantly in sperm rise and discontinuous percoll gradient density method (p<0.05). ther recovery rate of sperm was the highest in discontinous percoll gradient density as 66.84 +- 11.55%, the lowest in swim up method as 21.60 +- 7.83 %. The recovery rate of swim up, human serum albumin, bovine serum albumin, sperm rise, discontinuous percoll gradient, continous percoll gradient method were 21.60 +- 7.83 % 29.65 +- 12.0 % 24.40 +- 9.10% 61.37+-6.72% 66.84+-11.55% 54.79+-6.35% each, The recovery rates of the first three were significantly lower than latter (p<0.05). From the above results, sperm preparation can be useful methods for obtaining motile spermatozoa fraction and may be used in intrauterine insemination procedure. It is considerd that percoll gradient density methods can be used X-linked recessive familly for gender selection. In certain cases of male preponderance, swim up or albumin gradient method may be carefully tried to get male sex.
