본 가톨릭의과대학 산부인과학교실에서 14례의 난소암환자에서 1차수술후 1차선 및 2차선 확학요법제 투여후 2차 및 3차 수술을 시행하여 다음과 같은 결과를 얻었다 1 환자의 연령분포는 30세부터 70세였고, 40세부터 50세 사이가 9예였다 2 병기별 분포는, 제 1기와 제 2기가 각각 2예 및 4예였고, 제 3기가 8예였다 3 병리조직학적으로는 serous cystadenocarcinoma가 10예, endometrioid carcinoma가 2예였고, mucinous cystadenocarcinoma와 분류안된 adenocarcinoma가 각각 1예였다 4 2차 개복수술의 결과는 무병상태가 4예, 육안적 유병상태가 10예이었고, 간전이가 1예이었다 5 2차 개복수술후 잔존 암종괴의 크기는 13예에서는 1 5cm이하로 제거되었고, 1예에서는 1 5cm이상의 종괴가 남았다 6 3차 개복수술을 시행한 2예의 수술 소견은 무병상태가 1예이었고, 육안적 유병상태가 1예이었다 7 1차 수술후, 2예는 12개월동안 추적하였고, 5예에서는 1년에서 2년간 추적하였으며, 4년에서 5년까지 추적하였던 경우는 2예이었고, 2년과 3년, 3년과 4년 사이는 각각 1예씩이었으며, 3예는 추적이 불가능하였다 8 총 14예중 1예는 간전이로 사망하였고, 3예는 계속 화학요법중이며, 7예는 임상적 추적관찰중이다 9 3차 개복수술은 2예에서 시행 하였으며, 1예는 임상적 관찰중이며 1예는 화학요법제 투여중이다

There have been many studies on the possibility of utilizing immunological tumor markers as means of diagnosis and follow-up study in ovarian cancer; however, the sensitivity and specificity, which will make it possible to confirm the existence of minute tumor mass, has not been ascertained yet So, for the purpose of early detection of remission and recurrence of ovarian cancer following the initial operation, various methods-clinical evaluation, ultrasonography, radiological examination, and CT scanning- are widely used; but, detection of residual tumors lurking in the abdominal cavity is not easy From this view point, for the establishment of therapeutic plans based on the degree of remission, the extent of residual tumors and early detection of recurrence of the disease, following the primary operation, the importance of a second-and third-look evaluation can not be denied This study evaluated a second-and third-look operation on 14 cases with ovarian cancer following the first and second line chemotherapy at St Mary`s Hospital and Kangnam St Mary`s hospital with the following rewults: 1) Of the total 14 patients, ranging in age from 30 to 70 years, those who were at the age from 40 to 50 years numbered nine 2) By stage, eight were in stage III, and two in stage I and four in stage II 3) By pathologic type, 10 patients had serous cystadenocarcinoma, two endometrioid carcinoma, and one mucinous cystadenocarcinoma, and one unclassified adenocarcinoma 4) The findings of second-look operation revealed no evidence of disease in four patients, macroscopic evidence of disease in nine and liver metastasis in one 5) At the time of second-look operation, the size of remaining tumors measured less than 1 5cm in 13 cases, and more than 1 5cm in one case 6) The findings of third-look operations in 2 cases showed no evidence of disease in one and macroscopic evidence of disease in the other
