임산부 및 제대혈에서의 B형간염 표식자 발현양상
Hepatitis B Markers Expressed in Term Pregnant Women and Umbilical Cord Sera of Their Babies

1985년 1월 1일부터 1985년 12월 31일까지 본원 산부인과에 입원, 분만한 정상산모 763명과 HBsAg양성산모 및 anti-HBs 양성산모에서 출생한 신생아 중 부모가 본 조사에 참여키로 동의했던 104명의 신생아를 대상으로 RIA법을 사용하여 B형간염표식자를 검사, 아래와 같은 결과를 얻었다 1 763명의 산모중, HBsAg 양성은 6 55%(50명)였고, anti-HBs 양성은 30 67%(234명)였다 2 HBsAg 양성산모중, e-Ag 양성산모는 30 23%였고, anti-e 양성산모는 53 49%였으며, 이들에게서 출생한 신생아제대혈내 HBsAg 양성율은 32 26%였다 3 e-Ag 양성산모에서 출생한 신생아제대혈내 HBsAg는 75%에서 양성이었으나 anti-HBs는 검출되지 않았고, anti-e 양성산모의 신생아제대혈내 HBsAg는 28 57%에서, anti-HBs는 14 29%에서 양성을 나타내, e-Ag 양성산모에서 출생한 신생아에서 HBsAg 양성율이 의의있게 높았다 4 HBsAg 양성산모에서 anti-HBc 양성은 87 10%, 음성은 12 9%였다 산모가 anti-HBc 양성일 때 제대혈내 HBsAg는 37 04%에서 anti-HBs는 22 22%에서 양성이었고, 산모가 anti-HBc 음성일 경우 제대혈내 anti-HBs 양성은 75%로서 anti-HBc 음성산모에서 출생한 신생아의 anti-HBs 양성율이 의의있게 높았다 5 산모가 anti-HBs 양성의 경우, 제대혈내 anti-HBs 양성은 86 3%, anti-HBc 양성은 82 19%였다 6 간염기왕력이 있는 산모의 34 48%가 HBsAg 양성을 나타냈으며, 기왕력이 없는 산모군보다 의의있게 높은 빈도를 보였다 7 HBsAg 양성산모의 미숙아분만 빈도는 의의가 없었으나, 기형아분만은 의의있게 높은 빈도를 보였다

We studied the various hepatitis B markers in 763 healthy term pregnant women and in 104 umbilical cord sera of their babies born to confirmed HBsAg positive or anti-HBs positive mothers during the period from Jan 1 to Dec 31, 1985 The detection of hepatitis B markers was done using radioimmunoassay methods We performed the study to find the incidence of hepatitis B markers in maternal and cord sera, and to find the relationship between maternal hepatitis B markers and premature delivery or congenital anomaly in the newborns The results obtained are as follows; 1 Of 763 mothers, serum HBsAg was positive in 6 55%(50), and anti-HBs was positive in 30 67%(234) 2 Of HBsAg positive mothers, e-Ag was positive in 30 23% and anti-e was positive in 53 49%, and HBsAg was positive in 32 26% of the cord sera of the babies born to HBsAg positive mothers 3 Of mothers who were e-Ag positive, HBsAg was detected in 75% of their cord sera, whereas no anti-HBs was detected 4 The mothers with HBsAg positive sera were found to bear anti-HBc in 87 1% with no anti-HBc in 12 9% When the mothers were anti-HBc positive, HBsAg was positive in 37 04% with anti-HBs being positive in 22 22% of their cord sera On the countrary, when the mothers were anti-HBc negative, anti-HBs was foung 75% of their cord sera suggesting that anti-HBs positive rate is significantly higher in the newborns of anti-HBs negative mothers 5 When the mother were anti-HBs positive, anti-HBs positive rate was found to be 86 3%, while Anti-HBc positive rate was found to be 82 19% of the cord sera of their babies 6 Of the mothers with prvious history of hepatitis B, HBsAg was found to be positive in 34 48% of the cases, which is a significantly higher rate in comparision with those without a history of hepatitis B 7 There was an increased incidence of congenital anomaly in HBsAg positive mothers, but no relationship was found in the incidence of premature delivery
