임신성 고혈압질환에 있어서의 혈액응고계의 변화
A Study of Hematological Changes in Pregnancy Induced Hypertension

1984년 8월 1일부터 1984년 12월 31일까지 일신기독병원 산부인과에 분만 및 치료를 위하여 입원한 환자 중 임신성 고혈압 질환이 나타나는 산모 150명을 전자간 경증 60예 중증 72예 자간증 18예와 대조군 50예를 대상으로 임신성 고혈압 질환에 있어서의 혈액 응고계의 변화에 대하여 비교 측정하여 다음과 같은 결론을 얻었다. 1. 경증 전자간군: 응혈시간이 지연되었으며 섬유소원 분해산물이 의의있게 증가 되었다. 2. 중증 전자간군: 헤마토크리트치의 감소, 응혈시간의 지연, 섬유소원 분해산물의 증가가 나타났다. 3. 자간군: 응혈시간의 지연, 혈소판수의 감소, 프로트롬빈시간의 지연, 활성부분트롬보플라스틴시간의 지연, 섬유소원의 감소, 섬유소원 분해산물의 증가를 나타내었다.

During a 5 month period from August 1st 1984 to December 31st 1984 hematological studies were done on 150 patients with Pregnancy Induced Hypertension(P.I.H.) and in 50 women with normal pregnancies, who were admitted for delivery or for treatment of P.I.H. to the department of Obstetrics and Gynecology in Il Sin Christian Hospital. The purpose of the study was to note the differences the results of the hematological studies, particularly in coagulation factors, between patients with normal pregnancy and those with P.I.H., and also to note if there was a correlation between the severity of P.I.H. and results of hematological studies. The studies that were done were, Hemoglobin(Hgb.), Hematocrit(Ht.), bleeding time(B.T.), clotting time(C.T.), platelet count, prothrombin time, activated partial thrombopla stin time(A.P.T.T), fibrinogen level, and amount of fibrin degradation products(F.D.P.). The results were as follows: 1. Mild pre-eclampsia Clotting time was prolonged, and F.D.P. were increased. 2. Severe pre-eclampsia Hematocrit was decreased but as many of these patients had associated severe antepartum hemorrhage the result is probably not significant. 3. Eclampsia All clotting factors showed abnormal results; increased clotting time, decreased platelet count, prothrombin time and A.P.T.T. both prolonged, fibrinogen decreased and F.D.P. were markedly raised. 4. Fibrin degradation products This factor was the only one to show a progressively more abnormal result with increasing severity of pregnancy induced hypertension, and could be used as an indication of increasing severity of the disease.
