1983년 6월 1일부터 11월 30일까지 만 6개월간 본원 산부인과에서 정상 질식분만한 산모 27명을 대상으로 유즙분비를 억제할 목적으로 steroid 제제인 diethyl stilbestrol을 투여한 14예와 bromocriptine을 투여한 13예에 대하여 약제 투여후 임상적 효과와 혈 중 prolactin 치를 측정한 후 비교 관찰하여 다음과 같은 결론을 얻었다. Bromocriptin 투여 방법은 종래의 1일 5 mg 14일 투여방법에서 1일 7.5 mg씩 7일간과 5 mg씩 3일 투여로 10일간 투여하였다. 1. Steroid치료군에서 유즙분비는 산후 제 3일에 71.4%, 제 5일에 28.6%, 제 10일에 14.3%였다. 2. 이에 비하여 bromocriptine 치료군에서는 유즙분비가 제 3일에 0%, 5일과 10일에 7.7%를 나타내어 steroid 투여군보다 현저한 유즙분비 억제효과를 나타내는 것을 알 수 있었다. 3. 부작용은 거의 없었으며 단지 2예에서 투여후 제 14일에 반동성 유즙분비를 보였으나 1주간의 추가투여로 증상이 소실되었다. 4. 혈중 prolactin치는 bromocriptine치료군에서 steroid 치료군보다 급격히 낮아져 산후 제 2일에 41.0+-34.7 ng/ml로 비임산부의 수준을 유지하였다.
A comparative clinical study on the effectiveness of bromocriptine and steriod hormone in suppressing puerperal lactation was carried out in 27 cases of puerperal women evaluating clinical findings of milk secretion, breast engorgement and pain, and determinging serum prolactin levels. the bromocriptine group of 13 cases received 2.5 mg of bromocriptine three times a day for 7 days and followed by twice by twice daily for 3 days, total 67.5 mg for 10 days and steroid hormone group of 14 cases received 5mg of diethylstilbestrol three times a day for 5 days. The radioimmunoassay of serum prolactin levels in 13 bromocriptien treated women were so impressive as to fall rapidly down to the normal levels within approximately 2 days, but steroid treated groups were above 200 ng/ml during the 1st postartum week. Criteria of clinical evaluation were the degree of clinical evaluation were the degree of milk secretion, breast engorgement, pain and tenderness, and rebound lactation. In the steroid group, milk secretion was observed in 71.4% of the patients on the 3rd day and 28.6% on the 5th day whereas none of the cases on the 3rd day and 7.7% on the 5th day in the bromocriptine group. Also in the bromocriptine group rebound lactation was observed in 15.4%, but it was mild and stopped in all with further treatment for 1 week in spite of relative short term therapy for 10 days. In conclusion, short term therapy of bromocriptine was significantly better than steroid therapy.
