1981년 1월부터 1981년 6월까지 경희대학교 의과대 학 부속병원 산부인과에 내원한 경산부 354명을 대상 으로 양수내 AFP치를 방사면역측정법으로 측정하였으 며,이중 단일정상아를 분만한 273명을 정상군으로 하 고,태아의 신경관절손증이 확인된 임부 7례에서도 양 수내 AFP치를 측정하여 다음과 같은 결론을 얻었다. 1.정상 한국중기임부의 양수내 AFP치는 15~16주 에서 평균 16.9㎍/㎖,17~18주에서 평균 17.1㎍/㎖, 19~20주에서 평균 16.3㎍/㎖,21~22주에서 평균 15. 9㎍/㎖,23~24주에서 평균 13.7㎍/㎖이었다. 2.15~18주에서 양수내 AFP치는 외국임부의 수치 와 유사하였으나,19~24주에서는 외국임부의 수치보 다 현저하게 높아서 1.3~3.0배정도이었다. 3.신경관결손증이 합병된 20주이전의 2례에서 양수 내 AFP치는 정상군과 비교하여 20~30배정도의 현저 한 증가를 보였으며,27주이후의 5례에서는 동일임신 주수의 외국임부와 비교할 때 모든 례에서 95th centile 이상의 증가한 소견을 보였다. 4.정상 중기임부중 양수내 AFP치의 가양성을 보인 례는 273명중 6례로서,빈도는 약 2%이였다. 5.쌍태아임부 2례에서 양수내 AFP치는 정상이었다. 결론적으로 단순복부X선촬영 혹은 초음파검사로 신 경관결손증을 쉽게 진단할 수 없는 20주이전의 초기임 부에서 양수내 AFP치를 측정하는 것은 신경관결손증 의 조기산전진단에 큰 가치가 있을 것으로 사료된다.

During early fetal life,human alpha-fetoprotein(AFP)is the ajor serum pr- otein of the fetus,and found in the amniotic fluid on maximal concentration at the 12 to 14 weeks of gestation on normal pregnancy. Increased amount of AFP was reported to be found in pregnancies with neural tube defects.Amniotic fluid AFP levels were measured by radioimmunoassay from 273 normal pregnancies and 7 complicated pregnancies with neural tube defects dur- ing January,1981 to July,1981 t Kyung Hee University hospital. The following results were obtained. 1.Men amniotic fluid AFP values in Korean midpregnant women were 16.9 ㎍/㎖,17.1㎍/㎖,16.3㎍/㎖,15.9㎍/㎖ and 13.7㎍/㎖ in 15~16 weeks,17~18 weeks,19~20 weeks,21~22 weeks and 23~24 weeks respectively. 2.Amniotic fluid AFP values during 15~18 weeks were similar to the values of foreign studies,but the values during 19~24 weeks were higher. 3.In 2 pregnancies below 20 weeks complicated by neural tube defects,ammi- otic fluid AFP values were markedly elevated to normal range(20~30 times).In 5 pregnancies above 27 weeks,AFP values were also increase to the foreign Studies of same gestational period. 4.There were 6 false positive results from 273 normal pregnancies(about 2%). 5.In 2 twin pregnancies,amniotic fluid AFO values were all within normal ranges. In conclusion,the measurement of amniotic fluid AFP levels from pregnancies below 20 weeks of gestation would be valuable to detect the possible fetal neural tube defects.
