1.159명의 IUGR 신생아중 비대칭성 IUGR은 19명(11.9%)이었고 대칭성 IUGR는 140명(88.1%)이었다. 2.IUGR을 가진 신생아의 체중과 가장 밀접한 상관관계를 보인 것은 흉위였으며 복위, 두위, BPD 순으로 상관관계도가 저하하였다. 3.두위 및 BPD는 대칭성과 비대칭성 IUGR군에서 양군 모두 정상군에 비하여 유의한 감소를 보였으며(p>0.05) 양군 상호간에는 차이가 없었다.(p>0.05). 4.흉위 및 복위는 양군에 있어서 정상군에 비하여 유의한 감소를 보였고(p<0.05) 비대칭성군에서 대칭성군에 비하여 유의하게 감소된 차이를 보였다(p<0.05). 5. 이상과 같은 결과로 비대칭성 IUGR군에서의 brain sparing 개념은 잘못되었으며 비대칭성을 나타내는 이유는 brain sparing이 아니고 복부장기 발육장애로 인한 것으로 해석되며 BPD의 초음파 측정이 비대칭성 IUGR의 진단에도 도움이 된다.

A Clinical Comparison of Symmetric Versus Asymmetric Intrauterine Growth Retardation Kye Y.S.,M.D., Moon H.,M.D., Kim D.S.,M.D. Department of Obstetrics & Gynecology, Han Yang University, College of Medicine Seoul, Korea Controversy has been encounterred for the "brain sparing" concept and the value of ultrasonic measurement of BPD in detecting asymmetric intrauterine growth retardation. This study is a trial to confirm the brain sparing effect and reevaluate the value of BPD in diagnosing the asymmetric IUGR infants by comparing the anthropometric measurements between symmetric and asymmetric IUGR infants including BPD, head circumference, abdominal circumference, chest circumference, and head circumference/abdominal circumference ratio. One hundred fifty nine IUGR infants delivered at the Department of Obstetrics and Gynecology, Han Yang University Hospital during one year from Jan. 1 to Dec. 31, 1981 were classified to asymmetric IUGR as the head/abdominal circumference ratio ranges more than +2SD and symmetric ones as +2SD or less. The anthropometric measurements of the IUGR infants were plotted on the corresponding nomograms and compared between the 2 groups. The results were as follows: 1.One hundred and fifty nine IUGR infants were classified to 19 (11.9%) asymmetric and 140(88.1%) symmetric type. 2.The mean percentage decreases of head circumference or BPD were similar between asymmetric and symmetric IUGR group (p > 0.05) and statistically significant compared to nomograms of both groups.
