Management of Premenopausal Bleeding

It is well known that menstrual irregularity become increasingly common in women towards the menopause. By far highest proportion of patients who bleeds abnormally after the age of 40 years are so-called functional bleeders. But in span of years between 40-years old and the menopause the likelihood of occurrence of uterine cancer rises abruptly. So we must keep in mind that malignant as well as dysfunctional bleeding is common in this period before accepting a diagnosis of benign functional bleeding. According to our data causes of abnormal vaginal bleeding in this period were as following order ; functional uterine bleeding (30.5%) infection (19.2%), myoma and adenomyosis (23%) and carcinoma of the cervix(6%). We usually performed curettage including Pap. Smear and possibly biopsy of cervix primarily to rule out uterine cancer, secondarily to stop bleeding . The perplexing hormonal dynamics of premenopausal woman were also reviewed.
