본병원 산부인과에서 37세된 경산부에서 파열된 과립 세포종 1례를 경험하였기에 문헌고찰과 함께 보고하는 바이다.

Ovarian granulosa cell tumor associated with rupture and shock is one of the extremely rare cases. The case presented is a 37 year old multipara with chief complaint of sudden low abdominal pain, rigidity, vomiting as well as profound shock due to ruptured large granulosa cell tumor of right ovary. There was no any episodes of vaginal spotting or discomfort except some occasional palpable mass in her low abdomen until one day prior to her admission to the hospital for the emergency state. The patient underwent emergency laparotomy and the tumor was removed. The abdomen was full of blood and it was estimated 3,000cc. The additional elective total hysterectomy and contralateral salpingoophorectomy were performed late in 2 weeks. The large granulosa cell tumor measuring 20×15×10cm in diameter was found to be twisted twice in its pedicle. The rupture of the tumor with heavy hemorrhage apparently resulted in secondary to the torsion. The pathologic finding revealed (1) a benign tubular pattern of granulosa cell tumor of the ovary (2) ruptured and twisted tumor. Even though the clinical picture of such case is very much similar to those of ectopic gestation or malignant tumors, I should be excluded adequately for the definitive treatment.
