Background: Patients with atopic dermatitis (AD) are vulnerable to xerosis due to impaired skin barrier function, which makes moisturizing essential. Recently, zinc-alpha-2 glycoprotein (ZAG) has appeared to modulate the skin barrier function in AD, and has been proposed as a potential therapeutic molecule in AD.
Objective: This study aimed to evaluate the effects of a ZAG-containing moisturizer on restoration of the skin barrier and clinical improvement of AD.
Methods: In this randomized, double-blind study, 42 patients (average age, 26.5 years) with mild-to-moderate AD were enrolled. The subjects were divided into two groups, ZAG-containing moisturizer or control, in which the intervention or control were applied twice a day for 4 weeks. The primary outcome was a change in the eczema area and severity index (EASI) after 4 weeks, and the secondary outcome included the transepidermal water loss (TEWL), corneometer, visual assessment score (VAS) for pruritus and sleep disturbance, and investigator’s global assessment (IGA).
Results: ZAG-containing moisturizer was well-tolerated, with a significant decrease in the EASI score compared to the control group after 4 weeks of application (p<0.05). As objective assessments of skin barrier function, TEWL also showed a significant, rapid decrease in the ZAG group compared to the control group after 2 weeks of application (p<0.05). A significant improvement in AD symptoms was observed at 4 weeks, such as the VAS score for pruritus and sleep disturbance, and IGA.
Conclusion: The moisturizer containing ZAG (By the doctor Ato repair cream, Whitecospharm, Korea) effectively restored the function of the skin barrier, which led to a relief in the signs and symptoms of AD. (Korean J Dermatol 2020;58(9):575∼582)

Background: Merkel cell carcinoma is an uncommon primary cutaneous neuroendocrine cancer. It is a highly aggressive cancer with high rates of local recurrence and nodal metastasis. While there are some case reports on Korean patients with Merkel cell carcinoma, there has been no comparison study between Western patients and Korean patients regarding its clinical features.
Objective: This study aimed to identify the clinical features of Merkel cell carcinoma in Korean patients and compare them with those seen in Western studies.
Methods: We retrospectively reviewed the medical records of patients who were diagnosed with Merkel cell carcinoma between January 1995 and May 2019. Clinical features were compared with those seen in Western studies.
Results: Thirty-one patients were enrolled in the analysis. The mean age of onset was 67.6 years, and there were more female patients (1:1.58). The head and neck was the most common primary site (38.7%, 12/31). Patients treated by surgical methods alone were the most common (58.1%, 18/31). Twelve patients (38.7%) had recurrence, and seven patients (22.6%) died of Merkel cell carcinoma. Patients younger than 70 years were more frequent in Korea than in Western countries (Fishers exact test, p<0.05). In addition, patients with distant metastasis were less frequent in Korea than in Western countries (Fishers exact test, p<0.05).
Conclusion: Compared with Western studies, there were no differences between demographic and clinical features, except that older patients and patients with distant metastasis were less frequent in Korea. (Korean J Dermatol 2020;58(9):583∼589)

Background: Despite the increasing number of syphilis cases, there has been a considerable lack of recent data comparing the efficacy and duration of benzathine penicillin G, ceftriaxone, and tetracycline in Korean patients.
Objective: To compare the efficacy and duration of benzathine penicillin, ceftriaxone, and tetracycline in Korean patients with syphilis.
Methods: We retrospectively reviewed 145 cases of syphilis from 2004 to 2019 and statistically analyzed the treatment efficacy and duration.
Results: Patients in the benzathine penicillin, ceftriaxone, and tetracycline treatment groups showed no significant statistical difference in terms of treatment rate (chi-square test, p=0.962). Similarly, treatment duration among groups was not statistically significant (one-way analysis of variance, p=0.792). Co-infection with human immunodeficiency virus resulted in reduced treatment rate (p=0.016) and increased treatment duration (p=0.007).
Conclusion: This retrospective study showed no significant difference between penicillin, ceftriaxone, and tetracycline in terms of treatment rate and duration. However, a difference in treatment rate and duration was evident between the non-infection and HIV co-infection groups. (Korean J Dermatol 2020;58(9):590∼595)

Background: Tumoral calcinosis is characterized by the deposition of calcific masses around major joints, and it often causes significant impairment of joint function. Dermatologists sometimes encounter patients with a subcutaneous hard mass around the joint. However, there are few studies about tumoral calcinosis in the dermatologic literature, especially in Korea.
Objective: The aim of this study was to determine the clinical characteristics of tumoral calcinosis.
Methods: We reviewed the clinical photographs, medical records, and biopsy specimens of 11 cases of tumoral calcinosis seen at our clinic in 10 years.
Results: All 11 patients were female, and the mean age at onset was 58.5 years. The mean duration of the disease was 7.1 years. Most patients (9, 81.8%) presented with an asymptomatic subcutaneous hard mass around the iliac crest. None of the patients had a family history, or abnormal serum calcium, phosphorus, and parathyroid hormone levels. All patients underwent surgery for treatment.
Conclusion: Tumoral calcinosis can occur sporadically without metabolic disease. Therefore, careful history taking and biochemical work-up involving the metabolism of calcium and phosphorus should be performed. If there is no peculiarityon examination, the lesion is cured by resection. Although the sample size of this study is small, it can be inferred that the characteristics of tumoral calcinosis in Korea can be described by its predominant sex preponderance and location, that is, the female sex and the iliac crest, respectively. 5A Gain-switched 311-nm Titanium:Sapphire Laser Treatment in Palmoplantar Pustulosis

Background: Palmoplantar pustulosis is a specific form of localized pustular psoriasis occurring on the palm and sole. Multiple therapeutic options, including topical and systemic agents as well as phototherapies, are available for palmoplantar pustulosis; however, treatment outcomes are not satisfactory in most cases. Recently, a gain-switched 311-nm Ti:Sapphire Laser was developed and showed good treatment response in vitiligo and atopic dermatitis.
Objective: To investigate the efficacy and safety of the 311-nm Ti:Sapphire Laser in the treatment of palmoplantar pustulosis.
Methods: A total of 24 patients with palmoplantar pustulosis were treated with a 311-nm Ti:Sapphire Laser twice a week for up to 32 sessions and had a 3-month follow-up visit. The treatment dose started at 300 mJ/cm2 and was increased by 50 mJ/cm2 at each subsequent session. The Palmoplantar Pustular Psoriasis Area and Severity Index score, 5-grade patient satisfaction score, and adverse events were evaluated.
Results: The mean Palmoplantar Pustular Psoriasis Area and Severity Index score decreased from 8.31±3.31 at baseline to 4.75±2.70 at 16 sessions, 3.26±2.18 at 32 sessions, and 4.05±2.19 at follow-up visit. In the subgroup analysis, non-smokers and emollients user groups showed better responses in Palmoplantar Pustular Psoriasis Area and Severity Index (p=0.033 and p=0.027, respectively). Adverse effects, including burning sensation and transient erythema, were limited and well-tolerated.
Conclusion: The 311-nm Ti:Sapphire Laser can be considered as a treatment option for palmoplantar pustulosis. Moreover, habitual risk factor modifications, such as smoking cessation and steady use of emollients, can impact treatment outcomes in patients with palmoplantar pustulosis. Background: Few studies have investigated the association between Behçet disease (BD) and cardiovascular disease (CVD). The aim of this study was to investigate the risk of various CVDs in patients with BD.
Objective: The aim of this study was to investigate the risk of various CVD in patients with BD.
Methods: Between 2003 and 2015, we performed a retrospective cohort study involving patients with BD selected from Korea's National Health Insurance Service-National Sample Cohort database and age- and sex-matched controls. Age- and sex-matched controls were selected randomly from the NHIS-NSC database at a frequency of 1:5.
Results: Among the 998 patients with BD and the 4,990 controls studied, patients with BD showed significantly higher risk for angina pectoris (adjusted Hazard Ratio [HR] 1.522, 95% confidence interval [CI] 1.020∼2.273; p=0.04) and peripheral arterial disease (adjusted HR 2.939, 95% CI 1.296∼6.664; p=0.01) than the controls. The cumulative incidence rates of these diseases in patients with BD were also significantly higher than those in the controls.
Conclusion: Patients with BD showed independent risk for angina pectoris and peripheral arterial disease. (Korean J Dermatol 2020;58(9):608∼613)

Kawasaki disease is an acute inflammatory syndrome affecting middle-sized arteries. It is diagnosed when a fever lasts longer than 5 days, and four out of the five diagnostic criteria are satisfied. Some skin lesions are not included in the criteria, but are associated with Kawasaki disease. They can be helpful when the patient has an incomplete presentation. Herein, we report a Kawasaki patient who presented with simultaneous orange-brown chromonychia on all 20 nails and black crusted lips. (Korean J Dermatol 2020;58(9):614∼616)

A poroma is a benign adnexal neoplasm originating from the sweat gland duct. It usually presents clinically as a soft erythematous or flesh-colored papule, plaque, or nodule on the palms and soles. In most cases, poromas manifest as a solitary lesion, but rarely, multiple lesions have been reported and are defined as poromatosis. Although the pathogenesis is unclear, poromatosis is known to be associated with actinic damage, human papillomavirus infection, radiation therapy, or polychemotherapy. Herein, we describe a 53-year-old woman who had multiple erythematous papules on her feet for a year. She had a medical history of acute lymphoblastic leukemia and had undergone polychemotherapy with stem cell transplantation. To our knowledge, poromatosis related to chemotherapy is a rare entity, and our case could be attributed to the theory that using polychemotherapy can induce poromatosis. Prurigo pigmentosa is a rare inflammatory disease of the skin characterized by the unexpected onset of diffuse erythematous papules and macules on the chest, neck, and back, that usually resolves leaving reticular hyperpigmentation behind. Rarely, vesicular or bullous forms have been reported. We present two cases of vesiculopapular prurigo pigmentosa in two women. Two 20-year-old women presented with multiple, pruritic, erythematous vesicles and papules of various sizes on their backs for 4 months and 3 weeks, respectively. One remembered having a similar vesicular eruption 2 years ago. Histological study and clinical findings showed a vesiculopapular prurigo pigmentosa. Treatment with doxycycline 200 mg/day and topical steroid lotion showed good responses. The lesions resolved after leaving a light brown reticulated hyperpigmentation behind. Herein, we report two cases of vesiculopapular prurigo pigmentosa successfully treated with doxycycline and topical steroid lotion. (Korean J Dermatol 2020;58(9):620∼623)

Thymoma-associated multiorgan autoimmunity (TAMA) is a rare paraneoplastic disease defined as thymomas with liver, intestine, or skin manifestations, resembling graft-versus-host disease (GVHD) histopathologically in the absence of hematopoietic stem cell transplantation. A 76-year-old woman who had been diagnosed with invasive thymoma presented for evaluation of erythematous scaly papules across the trunk and extremities. Histopathological findings revealed parakeratosis, many apoptotic cells in the epidermal layer, and perivascular and interface dermatitis with lymphocytic infiltration. These pathologic alterations were similar to those observed in GVHD. Interestingly, she had never undergone allogenic stem cell or solid-organ transplantation. Based on her medical history and the presence of GVHD-like skin symptoms, we suspected that the cutaneous lesions may have been related to aggravation of the thymoma. We report this rare and interesting case for educational purposes because such a case has never been reported in Korea before. (Korean J Dermatol 2020;58(9):624∼627)
