Objectives : Warm needling is a combined treatment technique of acupuncture and moxibustion. In this study, we aimed to find out the components related with the thermal stimulation of the warm needling and to provide basic data for the guideline of the warm needling technique in the clinic. Methods : In this study, we measured thermal change of 3% agarose phantom embedding K-type thermocouples in depths of 0, 1, 2, 4, 8 and 16 mm. The warm needling was performed with acupuncture needles of various specifications (0.50×30, 0.50×40, 0.30×30, 0.30×40, 0.20×30 and 0.20×40 mm). A linear regression analysis was performed to find out the major component and quantify the effectiveness of the thermal stimulation during warm needling. Results : As a result of the measurement of temperature change, we could observe the thermal change pattern from the surface of the phantom to the 16mm deep part of the phantom. The thermal pattern was similar among the needles of different specifications. The regression analysis pointed the distance between the moxa cautery and the skin surface as the main component for the thermal stimulation of the warm needling. Conclusions : The authors suggest considering the distance between moxa cautery and the skin rather than the diameter of the acupuncture needle in accordance to the result of the study.

Objectives : This study aimed to investigate the effects of electroacupuncture (EA), the laser acupuncture (LA) and the combination therapy (LA+EA) in a collagenase-induced osteoarthritis rat model. Methods : Osteoarthritis rat model was induced by injection of collagenase into left lower articular cavity (50 μl to knee and 10 μl to ankle). In order to assess the anti-osteoarthritic effects of EA, the 650 nm LA and 650 nm LA+EA, the histopathological findings and plantar withdrawal responses were analyzed. Results : All of the treatment methods used in this study were effective in reducing pain. All treatment groups were effective in decreasing inflammatory cytokines of TNF-a and IL-6; the 650 nm LA and 650 nm LA+EA groups significantly reduced IL-1β. Conclusions : 650 nm LA and EA inhibit the production of collagenase-induced inflammatory mediators of osteoarthritis.

Objectives : The liver is rich in mitochondria and it plays a key role in whole-body energy homeostasis. Mitochondria is double membrane-bound organelle that supplies energy for intracellular metabolism including Krebs cycle and beta-oxidation. Acupuncture is known to stimulate and regulate the flow of energy. To explore the effect of acupuncture on the mitochondrial respiratory chain activity in the rats' livers, the activity of mitochondrial respiratory chain complexes I to IV was observed. Methods : The rats were divided into 4 groups; Normal 1 (no acupuncture treatment and anesthesia for 5 min), Normal 2 (no acupuncture treatment and anesthesia for 10 min), MA1 (acupuncture treatment at bilateral LR3 under anesthesia for 5 min), and MA2 (acupuncture treatment at bilateral LR3 under anesthesia for 10 min). All rats were sacrificed and the livers were examined for respiratory chain change. Results : There was no difference in ubiquinon oxidoreductase, succinate dehydrogenase, and ubiquinol cytochrome C oxidoreductase after acupuncture at LR3. Acupuncture at LR3 for 10 min increased the activity of cytochrome C oxidase compared with no acupuncture groups. Conclusions : Acupuncture at LR3 mediated mitochondrial respiratory chain activity via the cytochrome C oxidase signaling pathway in the livers of rats.

4구기자 복합물 약침액이 간기능 개선에 미치는 영향

Objectives : This study investigated the hepatoprotective effect of herbal mixture including Lycii fructus (HML) in thioacetamide (TAA)-induced hepatotoxicity in mice. Methods : To confirm the liver protective effect, induced by TAA for 3 days injection at 100 mg/kg mice, HML were treated for 8 weeks at 300 mg/kg/day, 1000 mg/kg/day. Positive control was treated silymarin 50 mg/kg/day after TAA injection. The changes of mortality rate, clinical signs, organ weight, relative liver, blood chemistry and histopathological findings were analyzed after experiment. Results : Body weight gain was observed in all groups, but TAA treated group at 4th week and all treated groups decreased weight compared to the untreated group. As a result of organ weight measurement, organ weight gain due to hepatic injury was observed statistically significantly in TAA-treated group and TAA+Silymarin treated group, and the herbal mixture-treated group showed a tendency to decrease compared to the TAA treated group. Blood biochemistry showed that total cholesterol and very low density lipoprotein cholesterol decreased statistically in TAA+low-dose and high dose herbal mixture treated group compared to the TAA-treated group. Histopathological examination showed that liver abnormalities were not observed in untreated group, liver fibrosis was observed in liver injury with TAA treated and herbal mixture treated group. And, TAA+high dose herbal mixture group showed relaxation tendency on liver calcification compared to the TAA treated group. Conclusions : According to the above results, HML provided hepatoprotective effects on the hepatic injury by reduction of inflammatory responses.

Objectives : This research was performed to investigate the effects of Hwangryunhaedok-tang decoction and Hwangryunhaedoktang Pharmacopuncture at BL10 on cognition and memory impairment in a mouse dementia model induced by scopolamine. Methods : Fifty ICR mice were divided into 6 groups : Normal group (n=5), Control group (n=9), Positive control group for pharmacopuncture group (n=9, Donepezil 0.75 mg/kg), Positive control group for oral administration group (n=9, Donepezil 5 mg/kg), Pharmacopuncture group (n=9, Hwangryunhaedok-tang Pharmacopuncture undiluted solution 10 ml/kg), and Oral administration group (n=9, Hwangryunhaedok-tang 200 mg/kg). For a mouse dementia model, 1 mg/kg scopolamine was intraperitoneally administered to ICR mice. Hwangryunhaedok-tang pharmacopuncture was administered on BL10 for 4 weeks at intervals of 2 days. Hwangryunhaedok-tang decoction was given orally for 4 weeks every day. Morris water maze and passive avoidance test were conducted followed by measurement of acetylcholine concentration, acetylcholinesterase activity, and the amount of BDNF and p-CREB in the brain. Results : 1. In the Morris water maze test, the time spent staying around the platform significantly increased in the pharmacopuncture group and oral administration group than in the control group. 2. In the passive avoidance test, the time spent in the bright room significantly increased in the pharmacopuncture group and oral administration group than in the control group. 3. The level of acetylcholine in brains increased in the pharmacopuncture group and oral administration group than in the control group. Also, the activity of acetylcholinesterase decreased in the pharmacopuncture group and oral administration group than in the control group. 4. The expression of BDNF and p-CREB decreased in the control group, but increased in the pharmacopuncture group and oral administration group. Conclusions : These results suggest that Hwangryunhaedok-tang decoction and Hwangryunhaedok-tang pharmacopuncture at BL10 may have cognition and memory-enhancing effect in scopolamine-induced dementia in ICR mice via controlling the content of acetylcholine and the activity of acetylcholinesterase, and activating BDNF and p-CREB.

Objectives : The purpose of this study was to examine the effects of electroacupuncture at GB14, on brain activity assessed on an electroencephalogram(EEG). Methods : (1) Thirty-four healthy participants were randomly divided into two groups, receiving either real acupuncture or non-penetrating sham acupuncture at GB14. (2) EEG measurements were conducted simultaneously before and after a 5-minute electroacupuncture stimulation, and the differences in the resulting EEG parameters were compared between the test and control groups. Results : (1) Absolute power increased significantly in the theta-wave channel of the occipital region and in the entire alpha-wave channel. (2) Relative power decreased significantly in the theta-wave channels of the frontal and occipital regions. (3) Coherence decreased significantly in the theta- and beta-wave channels of the parietal and occipital regions, and increased significantly in the alpha-wave channels of most areas. Conclusions : This study shows that electro-acupuncture stimulation at GB14 activates the alpha frequency band in particular.

Objectives : This research aimed to 1) analyze the true meanings of the chapter, 'Wigi Haeng (Wei Qi Xing), the movement of guard qi' in Youngchu (Ling Shu) from the point of view of ancient Chinese Astronomy, 2) calculate the speed of Wigi over 24 Chinese seasons, 3) analyze the true meaning of daytime and nighttime in the chapter. Methods : 1) The chapter 'Wigi Haeng' was analyzed using concepts of ancient Chinese astronomy, 2) the records of angular distances of 28 constellations in the Book of Han (Han Shu) were used to analyze the meanings, and 3) the records of lengths of daytime and nighttime in the Book of Hou Han (Hou Han Shu) were used to calculate the speed of Wigi. Results : 1) The author of the chapter 'Wigi Haeng' did not consider the irregularity in the angular distances of the 28 Chinese constellations (Su). 2) The commentary in the Huangjenaegyong Taeso (Huang Di Nei Jing Tai Su) about the constellations in the chapter is correct. 3) The speed of Wigi changes in daytime and nighttime depending on the seasons. 4) When the speed of Wigi increases in daytime, the speed in nighttime decreases, and vice versa. 5) The beginning of daytime in 'Wigi Haeng' is not the time of sunrise but the time of dawn (2.5 Gak before sunrise). The nighttime ends 2.5 Gak after sunset. Conclusions : 1) The chapter 'Wigi Haeng' demonstrates the ancient astronomical point of view on the universe and the movement of Wigi. The speed of Wigi is variable. 2) This chapter does not address the irregularity in the angular distances of the 28 Su. 3) More research is needed on the meaning of daytime and nighttime in 'Wigi Haeng'.

Objectives : Microglia play a crucial role in electroacupuncture (EA) analgesia on neuropathic pain. The role of chemokines in producing analgesic effects of EA, however, is largely unknown. In the present study, we investigated the role of chemokines in producing analgesic effects of EA in the neuropathic pain model. Methods : Sprague-Dawley rats were randomly assigned into three groups (anesthetized group (ANE), non-acupoint EA group (NAP), and ST36 - GB34 EA group (ACU)). Neuropathic pain was induced by tight ligation of L5 spinal nerve. Mechanical and thermal hypersensitivity of hind paw was tested. Western blot tests and immunofluorescence assay for C-C motif chemokine ligand 2 (CCL2) levels and microglia activation were performed on spinal cord L5/6. EA was treated once daily from the 3rd day after surgery for 5 days. Results : EA treatments applied to ST36 and GB34 significantly reduced both mechanical and thermal hypersensitivity after two and three times of treatment, respectively. While CCL2 expression significantly increased in neuropathic rats, it was significantly reduced in the ACU. In addition, co-localization of CCL2 and activated microglia significantly decreased in the ACU compared to those of ANE and NAP in the spinal cord L5/L6 dorsal horn. Conclusions : The present results suggest that EA applied to ST36 and GB34 modulates the reduction of CCL2 release from the injured neurons and consequently decreases microglia activation in the spinal cord. Regulation of chemokine induced spinal activation of microglia plays a key role in analgesic effects of EA in the rat model of neuropathic pain.

Objectives : This preliminary study investigated the effect of acupuncture treatment on short-term ketogenic diet-induced weight loss and glucose intolerance in mice. Methods : Six-week old male C57BL/6J mice were randomly assigned to 3 groups: normal, KD, and KD+ACU. All the mice except normal group were fed with ketogenic diet formula for 7 weeks and mice in KD+ACU group received acupuncture treatment three times a week. Body weights were measured three times a week, and glucose level was measured on week 1,3,5, and 7. Ketone level was measured on week 3,5, and 7. Results : Ketogenic diet showed short-term weight loss effect, however, acupuncture treatment did not affect on the weight loss. Ketone level was increased in KD fed mice compared to normal diet fed mice and the level was decreased in KD+ACU group on week 3. However, the change was not significantly different compared to KD group on week 7. Glucose intolerance was improved in KD+ACU group compared to KD group. Conclusions : Acupuncture treatment was effective in relieving glucose intolerance, and the results suggest that combining acupuncture treatment with ketogenic diet may complement each therapeutic intervention by improving glucose intolerance but not effecting on weight loss. This study provides meaningful evidence as a preliminary study of acupuncture treatment on ketogenic diet.

Objectives : As acupuncture treatment has been widely practiced in many countries around the world, clinical trials of acupuncture treatments also have become popular. The objective of the study was to explore the trends of research investigating the effect of acupuncture treatment in clinical trials using a bibliometric approach, a quantitative analytical methods. Methods : Publications related to clinical trials using acupuncture from 2000 to 2019 were retrieved from the Web of Science database. Extracted articles were analyzed in terms of publication year, country, journal, research area, organizations and authors. Trends in research on acupuncture in clinical trials were visualized using the VOSviewer program. Results : A total of 3,166 articles of acupuncture clinical trials published from 2000 to 2019 were identified and analyzed. The country producing the most articles in this field was USA followed by China, England, South Korea, and Germany. A network analysis based on the co-occurrence of keywords showed following three clusters: clinical studies, pain management studies, and methodology studies. Conclusions : This study provided a macroscopic overview of research in acupuncture clinical trials. These findings provide an expansive strategy for researchers in this field to cooperate with other researchers or organizations.
