Pleomorphic xanthoastrocytoma(PXA) belongs to a subcategory of World Health Organization(WHO) classification of tumors of the central nervous system.9) PXA locates typically superficually and affects young patients, who present most commonly with seizures. It was first described by Kepes in 19799,10) and in Korea by Park et al in 1982.23) The tumor has aggressive histological features, but relatively favorable prognosis. The published cases of these PXAs in Korea were reviewed including the author`s own two cases, which were treated by gross total resection. The major clinical, radiological and histopathological features of PXAs in Korean are discussed.

The pineal region tumor is one of the difficult ones to exposure and remove. Selecting an operative approach to the pineal region tumor involving the third ventricle requires an understanding of the microsurgical anatomy of the posterior third ventricle. There are variable surgical approaches to the pineal region. The approaches suitable for reaching the pineal region are the infratentorial supracerebellar, occipital transtentorial, posterior transcallosal, posterior transcortical and combined supra/infratentorial-transsinus approach. Considerations important in selecting one of these surgical approaches are reviewed from the anatomical viewpoint.

The subfrontal interhemispheric approach offers safe access to lesions at anterior cranial fossa, including tumors like suprasellar craniopharyngiomas and meningiomas, and vascular diseases like anterior communicating artery or distal anterior cerebral artery aneurysms. Anatomic preservation of the pituitary stalk, hypothalamic structure, perforating vessels, anterior communicating complex, the visual pathway, and the olfactory nerves is often possible. This article describes traditional and modified versions of subfrontal interhemispheric approach from skin incision through dissection to closure. And the authors also describes merits, handicaps and possible complications of this approach.

Smads are signal transducers for the members of TGF-beta superfamily. Understanding the function of Smads in tumor cells is of paramount importance, since most tumors with defects in TGF-beta signaling still have functional components of Smads pathways that may play critical roles in determining the malignant phenotypes. Of these Smads, Smad4 is essential for TGF-beta signaling. The purpose of this study was to elucidate TGF-beta 1 and Smad4 protein expression in human astrocytomas and its pathological significance by using immunohistochemical technique. A total 54 astrocytomas among the patients who underwent operation at Pusan National University Hospital between Jan., 1996 and Dec., 2001 were included in the study. Histologically the tumors were graded as three groups according to the WHO grading system and 15 cases were in grade II(low grade astrocytoma), 19 cases in grade III(anaplastic astrocytoma) and 20 cases in grade IV(glioblastoma multiforme). As a result, TGF-¥a1 expression tended to increase with increasing grade of malignancy. Expression of Smad4 was higher in grade II astrocytoma than grade IV astrocytoma. The univariate analysis showed that there was significant difference between the malignancy and expression of TGF-¥a1 and Smad4(p<0.05). In conclusion, it is presented that reduced Smad4 expression and high TGF-¥a1 expression might be significantly associated with increased malignancy and invasiveness of astrocytoma.

Object:Marrow stromal cells(MSCs) have been shown to have the capacity of orthodox and unorthodox plasticity. In this study, the authors tried to differentiate MSCs into immune effector cell and also to access in vitro cytotoxicity of MSCs from rat. Methods:Rat MSCs(rMSCs) were isolated by standard methodology and were activated by interleukin-2(IL-2), interleukin- 15(IL-15), colony stimulating factor GM-CSF, and combinations. Phenotypes of these various activated cells were determined using flow cytometry. The secreted protein from effector cells was estimated by enzyme-linked immunosorbent assay. Cytotoxicity of rMSCs and activated rMSCs against the target cells(9L rat glioma cell line) were estimated using visual survival cell assay. The expression of immune response related genes in activated cells was measured. Results:After various cytokines activation of rMSCs, the population of immune effector cells(CD8, CD161a) and immune reaction related proteins(IL-4, γ-INF) were increased. There was a significant cytotoxicity of rMSCs activated with various cytokine combinations. Apoptosis may be one of the lysis mechanism of target cells by activated rMSCs. The contributing genes could be γ-INF, FasL and perforin. Conclusion:This study suggests that rMSC may differentiate into immune effector cells and have cytotoxic capability against malignant glioma cells. However, we have to investigate orthotopic animal study for the proper translation.

Inflammatory myofibroblastic tumor(IMT) includes a diverse group of lesions characterized by inflammatory cell infiltration and variable fibrotic responses, which mimic neoplastic lesions. IMTs of the head and neck occur most commonly in the orbits, and their occurrence in the skull is rare. To our knowledge, only eight cases of IMTs of the skull(excluding cases in the eye and maxillary sinus) have been reported in Korea. We report a case of IMT involving the frontal bone. A 43-year-old man presented with right frontal pain. A right frontal scalp mass had been removed at another hospital one month ago, but the pain persisted. Brain computed tomography showed a suspected sclerotic lesion near the right coronal suture, with an enhancing epidural mass. A lesion of the frontal bone was removed by craniectomy. The cortical bone appeared pinkish-brown color. Dura mater adherent to the bone showed an inflammatory reaction and thickening and it was removed. Duroplasty and cranioplasty were performed. The pathology test indicated IMT. IMT must be distinguished from other infectious, granulomatous, and neoplastic lesions. It requires an aggressive approach including surgical intervention, with resection of all the bone and dura mater involved. We describe the diagnosis and treatment of skull IMT.

One of the major complications of craniotomy is hemorrhage. Postoperative hemorrhages usually occur at the site of the operation but can also occur remotely. Although there are few reports in the literature, remote intracerebral hemorrhages cause significant morbidity and mortality. Our report a case of catastrophic cerebellar hemorrhage after intraventricular tumor surgery. Relevant literature are reviewed and discussed.

Hemorrhage from meningioma is rarely reported. We report one patient with intratumoral hemorrhage with meningioma. A 43-year-old women presented with severe headache and right hemiparesis. Brain computed tomography and magnetic resonance images revealed a mass lesion with intratumoral hemorrhage in the left frontal lobe. Total removal of the tumor and hematoma could be achieved. The patient recovered uneventfully after the operation. Histologic findings are considered to be a atypical meningioma. The Ki-67 labelling index was high, which is considered proliferative property of meningioma. We review the relevant literature and discuss the possible mechanism of intratumoral hemorrhage in the meningioma.

This is a case of a 21-year-old woman who was treated primitive neuroectodermal tumor(PNET) when she was 12-yearold. At that time, we performed radical surgery, radiation therapy, and chemotherapy. Forehead mass has developed after 11 years. On computed tomography, high density mass was detected on frontal bone and this lesion was removed by surgical operation. After surgery, chemotherapy was done at two times. The histological diagnosis was postradiation osteosarcoma. Postradiation osteosarcoma is a rare complication of radiation therapy. It has histologically been high-grade and poor prognosis. However, complete remission was obtained in our case after a couple of surgical operation, chemotherapy. This is the first case of postradiation osteosarcoma to be reported in the domestic. We treated a patient who developed osteosarcoma in the frontal bone 11 years after radiotherapy for PNET in the cerebellum and achieved favorable outcome.

Spontaneous regression of intracranial tumor is rare and especially in case of intratumoral bleeding, it is hardly found in documented report. We experienced 34 year-old male patient with metastatic brain tumor from maxillary malignant melanoma which is turned into tumor bleeding, underwent rapid resolution of tumor bleeding. We report the clinical manifestation, radiological characteristics and histological aspect of this rare case with literature review.
