Giant hypertrophic gastritis is characterized by marked enlargement of the gastric mucosal fold and involves most often the greater curvature of the stomach, but also may involves the antrum. We report a case of giant hypertrophic gastritis on the gastric antrum with review of literatures. This patient was a 53 year-old female who complained of epigastric pain & discomfort for 3 years. Roentgenograms and endoscopy of the upper gastrointestinal tract demonstrated enlarged and distorted giant mucosal folds on the gastric antrum and the low body of the stomach. Subtotal gastrectomy and operative biopsy was done because of a clinical suspicion of malignancy.

Patients with chronic liver disease have an increased susceptibility to bacterial infection. Multiple factors may contribute to the susceptibility of bacterial infection in such patients. For evaluation of such factors, we have studied in polmorphonuclear leukocyte chemotaxis to N-formylmethionylleucylphenylalanine (f-MLP), serum complement components C3 and the immunoglobulin IgG, IgA and IgM in 8 acute B hepatitis, 7 chronic active hepatitis and 31 liver cirrhosis and PMN- leukocyte chemotaxis in 11 normal controls. The results were as follows: 1) The mean levels of immunoglobulins (IgG, IgA 8z IgM) were significantly increased in all three group than normal value (IgG 800-1,500mg/dl Ig 90-325mg/dl, IgM 45-150 mg/dl). The mean level of serum complement C3 and C4 were slightly decrease in liver cirrhosis (C3 44. 36±28.82, C4 19.69±12.88). 2) The mean numbers of chemotactic responses of polymorphonuclear leukocyte to f-MLP were significantly lower in liver cirrhosis, (92.68±38.88) and chronic aetive hepatitis (80.68±13.12) than that in normal controls (171.00±33.6) (p<0.05). Above results suggest that defective chemotactic response of polymorphonuclear leukocyte to f-MLP was potentially another contributing factor to increased host susceptibity to infection in chronic liver disease.

Adenosine deaminase (ADA) is an aminohydrolase that catalyses the deamination of adenosine to inosine in salvage pathway of purine metabolism, and highly important for proliferation and differentiation of lymphocytes. An increased enzyme activity in blasts, plasma or serum has been reported in various acute and chronic leukemias. Recently, several investigators have reported the clinical values of serum or plasma ADA activity in leukemias as a indicator of the leukemic cell burden in various disease statuses following therapy. The authors have investigated the clinical values of sereial determinations of serum ADA activity on 118 samples from 53 acute leukemia patients (40 cases of AML, 11 cases of ALL, and 2 cases of undifferentiated leukemia) in various statuses following chemotherapy. The results obtained are as follows: 1) The serum ADA activities of total patient group in initial status before chemotherapy, partial remission, and on relapse were 43.2±37.2U/L, 20.0±13.0U/L, and 42.2±29.1U/L, respectively, whieh were significantly higher than those of contol group (13.4±5.8 U/L); and in hypoplastic phase and complete remission those were 10.8±3.0U/L and 14.6+6.4U/L, respectively, which were not significantly differnet from those of control group. 2) The serum ADA activities in hypoplastic phase, partial remission, and complete remission were reduced significantly compared to those of initial status, and those on relapse elevated compared to those of all the statuses except for initial status. 3) In initial status, the ADA activities were remark- ably higher in ALL than in AML, but in partial remission, those were higher in AML than in ALL. 4) During complete remission, only two out of 32 assays (6.3%) showed serum ADA activity above the normal range (control mean+2 S.D.), while on relapse, 9 out of 13 assays (69.2%) showed above the normal value. 5) Infection had no influence on serum ADA activity in initial status of acute leukemias. 6) Only in ALL, some correlations were observed between the serum ADA activity and white cell count, percent of blast and blast count in peripheral blood, and percent of blast in bone marrow with correlation coefficient of equal to or above 0.7. In view of the above results, it is acertained that the serum ADA activity may reflect the leukemic cell bur-den in various statuses of acute leukemia following chemotherapy, and the serial determination of serum ADA activity may provide an useful supplemental guide for the judgement of complete remission.

This study was undertaken to investigate whether cell-mediated immunity are affected by renal diseases. Thirty six patients with lipoid nephrosis and chronic proliferative glomerulonephritis, and 16 controlled subjects were studied by summation of delayed-cutaneous hypersensitivity response to 8 test antigens such as Tetamas, Diphtheria, Group C-streptococcus, Tuberculin, Glycerim, Candida albicans, Tricophyton mentaprophytes and protes mirabilis. The total average diameter of all positive reaction in controlled subjects was 15.34±2.03 mm. Of the 16 subjects, negatvie reaction was observed in 3 (18.8%). In patients with lipoid nephrosis, during relapsing phase the total average diameter of all positive reactions was 5.38±2.48 mm. being significantly lower than that of normal subjects. Negative reaction was observed in 9 of 10 patients (90%). The total average diameter of all positive reactions in patients with lipoid nephrosis in the remission phase was 13.09±1.64 mm showing no significant difference in total average diameters between control and remission phase group. Negative reaction was observed in 1 of 9 patients (11.1%). In chronic proliferative glomerulonephritis patients with the nephrotic syndrome, the total average diameter was 10.25±2.20 mm, showing no significant difference with control group, Negative reaction was observed in 3 of 10 patients (30%). The total average diameters of all positive reactions in patients with chronic proliferative glomrulonephritis without nephrotic syndrome was 12. 93±3.48 mm, showing non significant difference in total average diameters between control and patients group. Negative reaction was observed in 2 of 7 patients (28.56%) This study suggests lipoid nephrosis impairs cell mediated immunity, and the impairement of delayed hypersensitivity reaction seems to be related to clinical stages of nephrosis.

The prick skin test and RAST were used commonly in demonstrating the presence of skin sensitizing antibody in the diagnosis of allergic diseases. The agreement rate and correlation between skin test and RAST were studied by using Phadebas RAST kits and Bencard allergen extracts of 37 common allergens. The following results were obtained; 1) Total agreement rate of 37 allergens between skin test and RAST was 62.3% in 1840 samples. 2) The positive rate of RAST was 20,3% in the + positive group of skin test in all allergens 34.4% in %, and 83.19o in %. 3) Correlation coefficient between skin test and RAST was 0.79 in D. pteronyssinus, 0.67 in D. farinae, and 0.55 in house dust. 4) RAST positive rate of positive skin test and RAST negatvie rate of negative skin test were 68.7%, 94.4% in D. pteronyssinus, 57.8%, 91.3% in D. farinae 47.5%, 96.1 % in Artemisia, 42.4%, 84.3% in Ambrosia, and 32.8%, 97.9% in house dust. 5) RAST positive rate of D. pteronyssinus, D. Farinae, house dust, cat epithelium, cockroach, Ambrosia, Artemisia, mold were 31.0%, 35.1%, 16.4%, 21.7%, 53.8 %, 10.0%, 13.3%, 47,1% in patients with asthma, 59.6%, 55.8%, 30.0%, 18.2%, 22.2%, 48.0%, 39.4%, 34.1% in allergic rhinitis, 60.2%, 56.9%, 37.4%, 34.1%, 40.0%, 21.2 %, 23.2%, 20.0% in asthma associated with allergic rhinitis, 6) The positive RAST rate of D. pteronyssinus was 88.1% when RAS1 of D. Farinae was positive, and positive RAST rate of D. farinae 88.8% when RAST of D. pteronyssinus positive.

The usual methacholine challenge procedures are quite time consuming. So the presumptive estimation of the provocation concentration from the percent changes of spirometric measurements was performed in 22 asthmatics, 17 allergic rhinitis pathents and 40 normal subjects. In asthmatics inhalation of 2mg/ml methacholine resulted in a 19.3% diminution in FEV1 and PC20 was estimated approximately as 2 mg/ml. But in allergic rhinitis patients and normal subjects PC20, were higher than 20 mg/ml because their responses to 20 mg/ml methacholine were -5.9% and -1.8%, respectively. The baseline lung function of responders (asthmatics in whom FEV, decreased by more than 15%) was lower than nonresponders (p<0.001), and a significant correlation was present between the baseline FEV, and % change FEV1 (r=0.46, p<0,05). These results suggested that PC20 is different according to the severity of the bronchial obstruction and it indicate the bronchial hyperreactivity of the present time. This simple, rapid method using the solutions of 2 mg/ ml and 20 mg/ml methacholine may give a rough but clinically useful information about the bronchial hyper- reactivity like as reactive to 2mg/ml, intermediate response, or nonreactive even to 20 mg/ml.

The efficacy of a highly purified, formalin inactivated hepatitis B vaccine (Hepavax-B®), 20ug of surface antigen (HBsAg), given at 0, 1, and 6 months, was evaluated in 83 healthy young volunteers (mean age;25 years old) according to the injection sites of vaccine. All three doses had been given by arm injections in 51 subjects (A group) and the remainders (22 subjects) had received buttock injections for the first 2 doses followed by third-dose arm injections (B group). A significant decrease in seroconversion was noted 6 months after first dose injections in B group male subjects (31%) comparing with B group females and A group males and females, but conversion rate was rapidly elevated (95%) shortly after third-dose arm injections in these subjects. Geometrical mean antibody levels expressed as ratio units (RU) were significantly low 6 months after first dose injections in B group (both female % male) (p<0.005 vs. A group). This difference between A & B group was disappeared by the rapid increase of antibody level in B group subjects 2 months after third-dose injections, but in comparison with females, mean antibody level of male subjects was still significantly low in B group (p<0.005). Overall antibody conversion rates were over 95%, and antibody levels were above 10 RU in 90% of responders in both A & B groups 2 months after third-dose injections. Subjects with antibody levels above 100 RU were 65% in A group responders at the same time. In conclusion, as a method of hepatitis B vaccination, arm seems to be a superior site of injection to buttock even in young persons whose expected antibody responses are known to be excellent.

Plasma amino acids were measured in 20 cases of liver cirrhosis only, 17 cases of patients with hepatic encephalopathy and 20 cases of normal person as a control. Among above groups of patients, CSF amino acid concentrations were measured in 15 cases of liver cirrhosis only, 17 cases of patients with hepatic ence-phalopathy and 15 cases of normal person. The plasma amino acid concentrations were determined by high performance liquid chromatography. The results were as follow: The plasma concentrations of branched chain amino acids (valine, leucine, isoleucine) were lower in cirrhotic patients and patients with hepatic encephalopathy (p<0.05) than control group and more decreased in patients with hepatic encephalopathy than cirrhotic patients. The plasma concentrations of phenylalanine in aromatic amino acids increased in hepatic encephalopathy (p<0.05), but tyrosine increased in both group of patients (p<0.05). 1n the study of CSF, there were no significant changes in branched chain amino acid concentrations, but in aromatic amino acid concentrations (phenylalanine, tyrosine) increased (p<0.05). In hepatic encephalopathy compare to liver cirrhosis only, phenylalanine concentration was higher (p<0.05) in CSF. The molar ratio between branched chain amino acids and aromatic amino acids (Val.+Leu.+Isoleu./Phen.+Tyr,) of plasma and CSF were lower in both groups of patients than control. In hepatic encephalopathy com-pare to liver cirrhosis only, the molar ratio was lower in hepatic encephalopthy.

It is evident that an elevation of urinary albumin excretion rate without clinical proteinuria strongly predicts a later progression of diabetic renal disease. Diabetic patients with normal or elevated resting albumm excretion rate show marked albuminuric response to physical exercise compared with matched non- diabetic controls. Employing treadmill test with exercise protocol of modified Bruce (stage II), we studied three groups: 7 normal controls, 11 type II diabetic patients (Group I) with normal urinary albumin excretion (AER<15 μg/ min) and 4 type 11 diabetics (Group II) with elevated urinary albumin excretion (AERa15 μg/min). The results are as follows: 1) In normal controls urinary albumin excretion rate was not increased after exercise. 2) In Group I patients (AER: 5.5±4.2 μg/min), urinary albumin excretion was increased to 7.8±10.8 μg/min after exercise. 3) In Group II (AER: 40.6±11.4 μg/min), urinary albumin excretion was increased to 92.0±35.8 μg/min after exercise, which was significantly higher than those observed in Group I. 4) Blood pressure and heart rate was not significantly different between normal controls and type II diabetic patients before and during exercise stress test. 5) There was poor correlation between diabetes duration or HgbAlc and maximal change in albumin excretion rate after exercise.

Diabetes mellitus has been clearly identified as an independent risk factor in coronary heart diasase, sudden death, congestive heart failure, stroke, and peripheral vascular disease. The diabetic patient is twice as prone to develop heart disease as the general population. Recently the term Diabetic cardiomyopathy has been appeared in medical publications. This term imply the existence of a specific diabetic heart disease, a new nosological entity that has been growing out of several studies performed during the last few years by research workers in diabetes. The assessment of left ventricular function as an index of potential myocardial involvement in diabetic patients without clinical evidence of myocardial ischemia or other cardiovascular abnormalities has become increasingly important. To examine left ventricular function in diabetic patients without clinical evidence of cardiac involvement, eleetrocardiogram, phonocardiogram, carotid pulse tracing, echocardiogram, apexardiogram, and the first derivative of the apexcardiogram were performed in normal subjects and 16 diabetic patients. The results were as follows. 1) Corrected R to first peak interval was 96.1±25.9 msec in diabetic group and 66.3±14.8 msec in control group (p<0.05), height ratio was 52.9±7.4% in diabetic group and 56.5±4.0% in control group (p<0.05). S to second peak interval was not significant value. 2) Pre-ejection period was 85.9±27.2 msec in diabetic group and 40.9±12.2 msec in control group (p<0.05), left ventricular ejection time was 302.5±39.9 msec in diabetic group and 340.0±20.7 msec in control group (p<0.05). Electromechanical systole was not significant value. 3) A-MVO was 90.4±19.1 msec in diabetic group and 64.8±13.7msec in control group (p<0.05), ejection fraction was 59.5±11.2% in diabetic group and 68.5±7.5% in control group (p<0.05). Based on these findings, mechanocardiography for assessment of left ventricular function may be utilized as a good index of potential myocardial involvement in diabetic patients.
