Multinucleate cell angiohistiocytoma is a rare benign fibrohistiocytic vascular condition with distinctive histologic features. It tends to occur more frequently in middle-aged women, to favor the extremities, and to appear as grouped reddish-brown papules. We describe a case of solitary multinucleate cell angiohistiocytoma that appeared as a papule on the upper lip of a 24-year-old man.

Background : Verruca plana is a common disease, which can cause substantial cosmetic problems. Traditional treatments are often ineffective and sometimes associated with complications. Objective:Our study was performed to determine the clinical effectiveness of Q-switched alexandrite laser for verruca plana. Methods:Eighteen patients with verruca plana were treated with Q-switched alexandrite laser (755nm wavelength, 100nsec pulse duration, 3mm pulse wide). Results:1. Treatments with fluences that ranged from 7.0∼7.5J/cm2 were given on average 1.6 times. 2. After 1 laser session,  75% of the lesions became clear in ten patients. 3. Hyperpigmentation occurred in three patients and had disappeared after a few months. Conclusion:The Q-switched alexandrite laser is an effective treatment modality for the verruca plana without adverse sequelae.

Background:Interleukin-8 (IL-8) is known to have an important role in the development of psoriasis. The relationship between the palmoplantar pustulosis and psoriasis is not clarified yet, but the common histologic finding, intraepidermal accumulation of neutrophils, suggests that IL-8 may have significant roles in the pathogenesis of palmoplantar pustulosis. Pompholyx showes spongiotic dermatitis and seems to have little association with IL-8 in its pathogenesis. Objective:We performed this study to elucidate the roles of IL-8 in the pathogenesis of palmoplantar pustulosis, psoriasis, and pompholyx. Methods: Immunohistochemical staining was done using frozen skin samples of 10 cases of each disease group and normal control. Results:Diffuse/strong IL-8 immunoreactivity was noted in palmoplantar pustulosis and psoriasis, whereas only faint/focal immunoreactivity was shown in normal control skin. Pompholyx showed moderate to faint immunoreactivity for IL-8. The scores of immunoreactivity in palmoplantar pustulosis and psoriasis were significantly higher than that of normal skin, but the scores of immunoreactiviy in pompholyx showed no statistically significant difference from those in palmoplantar pustulosis, psoriasis and normal control. Conclusion:Our results strongly suggest that IL-8 has significant roles in the pathogenesis of palmoplantar pustulosis as well as in psoriasis.

Background:Differential diagnosis of keratoacanthoma(KA) from squamous cell carcinoma(SCC) is often difficult, especially when SCC has KA-like features(KA-like SCC). A number of recent studies have been attempted to separate these two entities with the use of immunohistochemical stains. But the results were inconsistent and the studies with KA-like SCC are rarely reported. Objectives:The purpose of this study was to examine the expression patterns of p53 protein and Ki-67 antigen on KA and KA-like SCC using immunohistochemical staining method and to evaluate the usefulness of this method in distinguishing each other. Methods:We performed immunoperoxidase staining(LSAB technique) using monoclonal antibody to p53 protein(PAb1801) and Ki-67 antigen(MIB1) on the formalin-fixed, paraffin-embedded biopsy specimens obtained from 12 patients with KA, 8 patient with KA-like SCC, and 10 patients with well-differentiated SCC. Results:The results were as follows; 1) There was a significant difference in the p53 expression between KA(25%) and SCC group (KA-like SCC=88%, SCC=100%). 2) Mean Ki-67 labeling index was slightly higher for SCC group(KA-like SCC=30.72%, SCC= 31.23%) than for KA(25.30%), but this difference was not statistically significant. 3) In Ki-67 expression, KA showed more pheriperal basal pattern(91%), whereas SCC group showed more diffuse pattern(77%). Conclusion:Our results suggest that KA and SCC are distinct entities of different nature and that these immunohistochemical staining methods can be useful methods in differentiating KA-like SCC from KA.

Background:The CD44 antigen is a widely distributed cell surface glycoprotein implicated in multiple physiological cellular functions including cell-cell adhesion, cell-substrate interaction and cell activation. In the skin, CD44 is normally expressed in epidermal keratinocytes and hair follicular, sebaceous and eccrine epithelial cells. The precise functions of CD44 are not yet clearly understood, though they appear to be involved in the mechanism of tumor invasion and metastasis. Objective:The purpose of this study is to investigate the immunohistochemical expression of the CD44 molecule in the epidermal keratinocytic tumors with different biologic behaviors, such as squamous cell carcinoma(SCC),basal cell epithelioma(BCE), actinic keratosis(AK), bowen's disease, keratoacanthoma(KA), trichoepithelioma. Methods:Routine paraffin sections of formalin-fixed 33 tissues (15 SCC, 23 BCE, 9 AK, 7 KA, 7 bowen's disease, 5 trichoepithelioma, 2 psoriasis, 2 lichen planus) were labeled with anti-CD44 monoclonal antibody using avidin-biotin- peroxidase complex. Normal skin served as the negative control. Results:The results are as follows. 1. In SCC and KA, peripheral parts of the lesion were prominently stained. CD44v6 showed the strongest expression, followed by CD44s and CD44v4/5. The horn cyst and the keratin region of the horn pearl were not stained. The degree of staining was weaker in KA compared to SCC. 2. The degree of staining was weaker in BCC and TEE compared to SCC. The staining was more prominent at the center of the lesion than in the peripheral region. In TEE, stronger staining was found at the center or in the squamous epithelial region. 3. Among the seven cases of Bowen's disease, 4 cases were diffusely stained with all three CD44 types. In the other three cases, local staining with the standard type and diffuse staining with CD44v4/5 and CD44v6 were seen. 4. In AK, the early lesion showed decreased expression of CD44 in all three types. However, those with mature lesion and progression to SCC showed increased degree of staining in all three types of CD44. Conclusion:These results suggest that expression of CD44 in these keratinocytic tumors is not related to malignant transformation and metastasis, but instead may be related tumor differentiation.

Background:Although most dermatologic laser systems were developed to damage target tissue selectively, the epidermal injury has been a major problem in treating skin lesions of colored people. Recently, to minimize the epidermal damage during laser treatment, superficial cooling is being used. Cooling is one of the well known methods to reduce pain sensation. Objective:The purpose of this study was to evaluate the effectiveness of an air cooling in reducing pain associated with Q-switched Nd:YAG laser and pulsed dye laser treatment. Methods:We measured the temperature of normal skin during air cooling and studied 13 patients treated with a 1064 nm Q-switched Nd:YAG laser or 585 nm pulsed dye laser treatment with and without air cooling system. The degrees of pain reduction rates with and without cooling were assessed by the visual analogue scales. Results:1. It was shown that cold air flow from 2 cm distance for 10 sec decreased skin temperature about 10.6 ℃ to 17.2 ℃ depending on the body sites. 2. The average of the pain scores was 38.08±21.05 on the cooled site as compared to 70.77±15.76 on the uncooled site. Conclusions:The cold air decreased the skin surface temperature like the other epidermal cooling system and it was very effective to reduce the pain on the cooled site associated with dermatologic laser treatment.

7인설성 수부습진 병변의 각질층 세라마이드와 유리 아미노산

Background:Lipids of horny layer forming multiple lamellar structure in the intercellular space acts as a skin barrier having a primary protective function and keeps softness and flexibility of the skin by absorbing and maintaining moistures. Among the intercellular lipids, ceramides mainly contribute to this important role. In xerotic eczema, atopic dermatitis, psoriasis, ichthyosis, and experimentally induced scaly lesion showing dryness and scales, the amount of ceramides is decreased or distribution of ceramide is changed. In addition to lipids, free amino acids, a component of NMFs, serve as a water retainer and are decreased in ichthyosis or experimentally induced scaly lesion. Hand eczema has dry and scaly lesion showing impaired skin barrier and low water content. So, changes of ceramides or free amino acids can be considered in the lesion of hand eczema. Objective:The purpose of this study was to confirm the relationship between the development of hand eczema and changes of ceramides or free amino acids. Method:The lipids and free amino acids in scales from lesion of hand eczema were analyzed by using high performance thin layer chromatography and amino acid analyzer. Results:Amounts of total lipids extracted were 0.63±0.33㎍/cm2 in hand eczema and 0.44±0.26 ㎍/cm2 in control. There was no difference between the two groups. Cholesterol sulfate, glucosyl ceramide, cholesterol, triglyceride, sterol ester, and n-alkane showed no difference between hand eczema and control. But ceramides were significantly decreased in hand eczema(11.0±5.5%) compared with control(21.4±8.0%)(p<0.05). Especially, ceramide type IV was significantly decreased in hand eczema (6.6±5.3%) compared with control(15.6±6.2%)(p<0.05) but ceramide type III in hand eczema did not differ from control. Amounts of total free amino acids in 10mg of scale were 10.4±3.1nmol in hand eczema and 9.5±3.0nmol in control. There was no significant difference between the two groups. Conclusion:Dry skin and scales in hand eczema are related to the decrease of total amount of ceramides and ceramide type IV than amino acids in horny layer. To clarify the exact pathogenesis of hand eczema, further investigations on all types of ceramides and their defect in the process on biosynthesis of ceramides will be necessary.

Background:Fas and Fas ligand (FasL) are cell surface proteins that mediate apoptosis. Fas and FasL are expressed in normal epidermal cells, but are different in the expression patterns in epidermal layers.  Objective:The purpose of this study was to evaluate the expression of Fas and FasL in various skin diseases including non-tumorous diseases and tumors. Method:We performed immunohistochemical staining for Fas and FasL with monoclonal or polyclonal antibodies using frozen skin tissues from 29 patients. Results:1. In normal skin, Fas and FasL were expressed as intercellular and intracellular patterns. Fas was expressed in the basal and spinous layers, and FasL was expressed in the upper spinous and granular layers. 2. In psoriasis vulgaris and lichen planus, FasL expression was proportional to the degree of epidermal hyperplasia, such a relationship was not found in Fas expression. 3. In verruca vulgaris and herpes zoster, the upregulation of Fas and FasL expression was observed in lesional epidermis. 4. In skin tumors, Fas and FasL expression were observed in most tumor cells of Bowen's disease, keratoacanthoma, and squamous cell carcinoma. In basal cell carcinoma, FasL was expressed strongly and diffusely in the infiltrating tumor cells, but Fas was not expressed at all. Conclusion:Fas and FasL play an important role in the differentiating process of the epidermis by coordinate expression between them. In non-tumorous conditions and skin tumors, Fas and FasL might function as essential mediators of cellular turnover in pathogenesis of the skin diseases.

9고전적 Kaposi 육종 , 피부 전구암 및 피부암에서 Human Herpesvirus 8 DNA Sequence 검출

Background:Human herpesvirus 8 (HHV8) has been detected in all forms of Kaposi's sarcoma (KS). The role of HHV 8 in dermatologic diseases other than KS is controversial. Some studies based on polymerase chain reaction (PCR) findings suggest an association between HHV8 and epithelial tumors of the skin. Objective:To assess the presence of HHV8 sequences in classic KS and some premalignant and malignant cutaneous epithelial tumors by nested PCR and find out any variations in HHV8 DNA by direct sequencing. Methods:In total 71 sample tissues were obtained from Korean patients: 5 classic KS, 12 Bowen's disease (BD), 19 actinic keratosis, 14 squamous cell carcinomas and 21 basal cell carcinomas. The first and nested PCR were performed to amplify the fragments of HHV8 open read frame (ORF) 26 and ORF 75 which were directly sequenced by dideoxy termination methods. Results:HHV8 DNA sequences were found in 3 of 5 classic KS (60%) and in 1 of 12 BD. Other epithelial tumors were negative for HHV8. On sequencing PCR products, substitutions at 1033 C→T, 1086 C→T, and 1139 A→C were detected in ORF 26 of all 4 positive PCR products. In 2 of 3 KS positive for ORF 26, substitution was detected at 237 G→A in ORF 75. Conclusion:All of our Korean samples were included in the Zong's C strain. This study suggests that C strain is predominant in classic KS in Korea. But it is rare in non-KS cutaneous epithelial tumors. Studies of the length difference in the HHV8 genome should follow.

Background:The Fitzpatrick skin type has been a simple and useful method to categorize cutaneous sensitivities to ultraviolet radiation. Objective:In this study, we intended to know the distribution of Fitzpatrick skin type in Korean by interview method using questionnaire. Methods:A total of 1296 Koreans from various districts were included in this study. Each subject was interviewed about their skin reactions to ultraviolet radiation and determined their skin type by interview method. To evaluate the statistical significance, we used generalized logit model. Results:1. The order of frequency of skin type was type III(48.8%), IV(22.2%), V(17.8%) by the interview method, with the sun sensitive categories(type I and type II) reported for 11.2%. 2. The skin type III was the predominant skin type in both men and women. However, skin type IV was found more frequently in men and it was statistically significant. 3. There is a tendency that frequency of skin type IV and V were increasing with advancement of age. 4. The Fitzpatrick skin type has some relationship with each individual`s occupation, the degree of sun exposure and their own skin color in Korean. Conclusion:This study included relative large numbers of subjects from various districts in Korea. It can be used as basic data for various studies related to cutaneous reactions to ultraviolet radiation.
