Purposes: The aim of this study is to evaluate whether the contents of hospital reservation and reception applications(apps) are qualitatively useful in meeting the needs of medical consumers and improving hospital accessibility and convenience.
Methodology: (1) identify consumer needs through social data web mining, (2) describe the status of key contents of mobile apps to improve accessibility and convenience of care, and (3) verify the quality of apps through validated tools
Finding: The contents of 'mobile reservation function' and 'waiting time information provision' that can contribute to reduction of delay time of care and efficiency of desk work were supported, but the level of utilization was insufficient. The quality level of the app, including the level of consumers' needs, has shown a wide gap between the apps.
Implications: The recent development of mobile apps for hospital accessibility and consumer needs has shown a wide gap in the quality of apps, including information and aesthetic. Therefore, it is necessary to develop apps based on user interface(UI), user experience(UX) based designs that can promote the usefulness and convenience of apps while monitoring needs of consumers continuously

Purposes: This study has the purpose to the improvement of health promotion for local residents through delivery of high-quality medical service by improving imbalance of medical use and seeking an improvement plan for accessibility of effective medical service by understanding the pattern of medical use by region.
Methodology: As for the method, this study derived results at the significance level of p<0.05 through chi square test(χ² test) and Generalized Estimating Equation(GEE) SAS 9.4 version by using the data of the 7th Korean Longitudinal Study of Ageing 2018.
Findings: Study results show that local residents use medical service such as hospitalization care and outpatient visit more as compared to the residents in Gyeonggi-do/large or medium-sized cities. The more the number of chronic disease, the more they select hospitalization care rather than outpatient visit. Results also show that patients engaged in labour tend to select outpatient treatment rather than hospitalization treatment. Meanwhile, severity of disease turned out to be higher amongst medical care beneficiaries than that of national health insurance patients when comparing the types of medical security.
Practical Implications: In stead of solving the problem of the difference in medical use by region from the aspect of income and economic level, an integrative solution shall be provided putting viewpoint on the social phenomenon suited to the changes of the times. This study suggests a plan for using a health and medical community care center that acts as a gate keeper of regional medical service.

3치매환자 동거 가족의 미충족 의료 유형별 관련 요인

Purposes: Family living with dementia patients have the burden for caring and suffer from health problems. Therefore, proper supports for their health disorders are required. The purpose of this study with regard to this is to subdivide unmet healthcare needs of family living with dementia patients into affordability, accommodation, and accessibility and figure out the relevant factors.
Methodology: The 2017 Community Health Survey was used, and 2,331 families living with dementia patients was included. To figure out the factors with regard to the types of unmet healthcare needs, multinominal logistic regression analysis was conducted.
Findings: According to the analysis result, sex, age, monthly household income, economic activity, self-rated health, self-rated stress and perception of depressive symptoms turned out to be the factors related to unmet healthcare needs. Regarding affordability, unmet healthcare needs were low when the object was female, over 65, highly educated, and monthly household income were high. On the other hand, unmet healthcare needs was high when self-rated health was bad, self-rated stress was high, and had depression. With regard to accommodation, unmet healthcare needs were low when the object was over 65. Unmet healthcare needs were high when the object was female, economically active and had depression, and self-rated health was high. Regarding accessibility, unmet healthcare needs were low when the object was high school graduate, but it was high when self-rated health was bad.
Practical Implication: This study confirmed that the family with dementia patients had a high proportion of unmet healthcare needs due to affordability and accommodation. The existing main discussion was that the experience of unmet healthcare needs normally occurred due to economic reasons, but a consideration on various cases and factors is required to ultimately achieve the policy goal to reduce the unmet healthcare needs of the family living with dementia.

Purpose: The purpose of this research is to develop two medical tourism system models which explain medical tourism phenomenon with a systemic approach.
Methodology/Approach: This research was conducted using a qualitative data analysis which mainly refer previous references in relation to medical tourism in the areas of tourism and medicine. Leiper's tourism system model was utilized as a conceptual framework. In-depth interviews with experts in the area were attempted in order to pretest the models.
Findings: This research suggests a medical tourism system framework and a medical service provision framework. The first model presents medical tourism components and their relationships within a framework presented in a diagram. The second model shows the relationships among medical services required by medical tourists, the service providers, and service human resources along with movements of medical tourists.
Practical Implications: The first model presents a spatial composition of medical tourism components and their relationships, whereas the second model shows the linkage among medical services, the service providers, and relevant service human resources along with time sequential steps of medical tourists. These two models are complementary and may be used as useful tools to observe medical tourism phenomenon with a systemic and holistic approach. These two models may enable stake holders avoid unnecessary confusions and conflicts that result in duplication of government policies and a waste of budget and human resources.

Objectives: The purpose of this study is to evaluate the factors affecting patient satisfaction for the nursing service of comprehensive nursing care service unit in Tertiary hospital
Methods: This study employed a descriptive survey to investigate the nursing satisfaction and the study data was collected from 74 patients among 83 patient who were admitted to the nursing care integration service ward of an tertiary hospital for 10 days from May 9, 2018 to May 19, 2018. Data was analyzed by descriptive statistics, t-test, ANOVA, and multiple linear regression using SPSS 21 program.
Results: The average with nursing satisfaction was significantly different according to marriage type and KPCS level. It was found that physical factor and educational factor have statistically significant effect on nursing satisfaction. As physical care increase by one unit, patient satisfaction increase by 0.226(95% CI; 0.061-0.390) and also satisfaction increase by 0.443(95% CI; 0.070-0.816) as education care increase by one unit.
Conclusions: As a result of study, it was found that it is necessary to provide patients with more careful nursing service in physical and educational perspective, in order to improve the service quality of comprehensive nursing care service unit.

6병원근로자의 변화지향 조직시민행동 결정요인 : 직무만족, 조직몰입의 매개효과 검증

Purposes: This study aims to investigate the determinants of change-oriented organizational citizenship behavior(CO-OCB) of hospital employees and test the mediating effect of the job satisfaction and organizational commitment on the CO-OCB.
Methods: Data were collected from the self-administered questionnaire survey for hospital employees of ten hospitals located in five regions, including Seoul. Out of 1,100 questionnaires which were distributed through the administrative routes, a total of 617 copies were returned(response rate 56.1%) and analyzed by frequency analysis, t-tests, correlation analysis, multiple regression, and path analysis using SPSS 21.0 and AMOS 21.0.
Findings: The study results revealed that the explanatory power(56.4%) of the model was most significantly increased by the variables of psychological attributes(R2 -change 33,5%). Also, the variables of self-efficacy, followed by hope, the organizational climate for innovation, the psychological safety climate, team-member exchange, and leader-member exchange were found to have positive total effects on CO-OCB, through the mediating variables of job satisfaction and organizational commitment.
Practical Implications: The study results imply that, in order to improve the CO-OCB of hospital employees, the administrators have to take the integrated approach considering the various domains of factors including the psychological attributes, job characteristics, interpersonal relations, and organizational climate simultaneously.
