Models of traditional Chinese medicine (TCM) are still difficult to grasp from the view of a Western-cultural background. For proper integration into science and clinical research, it is vital to think “out of the box” of classical sciences. Modern sciences, such as quantum physics, system theory, and information theory offer new models, that reveal TCM as a method to process information. For this purpose, we apply concepts of information theory to propose a “Chinese black box model,” that allows for a non-deterministic, bottom-up approach. Considering a patient as an undeterminable complex system, the process of getting information about an individual in Chinese diagnostics is compared to the input-process-output principle of information theory and quantum physics, which is further illustrated by Wheeler's “surprise 20 questions.” In TCM, an observer uses a decision-making algorithm to qualify diagnostic information by the binary polarities of “yang” (latin activity) and “yin” (latin structivity) according to the so called “8 principles” (latin 8 guiding criteria). A systematic reconstruction of ancient Chinese terms and concepts illuminates a scattered scientific method, which is specified in a medical context by Latin terminology of the sinologist Porkert [definitions of the Latin terms are presented in Porkert's appendix [1] (cf. Limitations)].

2A Study on the Effectiveness of Korean Medicine Treatment for Patients' Involved in Traffic Accidents and the Analysis of Factors Affecting the Effectiveness of Korean Medicine Treatment - A Retrospective Review

Background: This study was conducted to verify the effectiveness and effectiveness-related factors of Korean medicine (KM) treatment for patients who have been involved in traffic accidents (TA).
Methods: The medical records of 150 TA patients hospitalized in Dongguk University Bundang Oriental Medicine Hospital from September 2017 to May 2018 were reviewed. Medical records included information on patient demographic characteristics, TA situations, use of KM treatment, clinical characteristics, 4 scales [numeric rating scale (NRS), visual analogue scale (VAS), EuroQol-visual analogue scale (EQ-VAS) and neck disability index (NDI)]. Statistical analyses were performed to determine the effectiveness and effectiveness-related factors of treatment.
Results: The 150 patients (almost 1:1 gender ratio) were typically in their 40's (26.67%), had rear impact to their vehicle (37.33%), were in the driver's seat (70.00%), and without head trauma and loss of consciousness (83.33%). The main symptoms were neck pain (60.66%) and lower back pain (54.00%). All 150 patients showed significant improvement after KM treatment. 4 scales significantly improved when the treatment started within 3 days from the accident and patients were hospitalized longer and took more diverse treatments. The patients experiencing a higher initial pain level (NRS 8-10, VAS ≥70) showed significant improvement of NRS and VAS scores with faster and longer inpatient treatments than the other patients. The patients with higher initial pain levels (VAS ≥70) also showed significant improvement in EQ-VAS scores.
Conclusion: KM treatment was effective for TA patients in this study. Treatment initiation time, hospitalization period, treatment diversity and initial pain level are related to the degree of improvement of a patient's symptoms.

3The Stomach's Communication with Its Related Acupoints, and the “Intelligent Tissue” Hypothesis

Background: The intelligent tissue hypothesis holds that an organ's states (be it normal function or stressed states) are reflected in real time at its related acupuncture points (acupoints), causing physical, real-time changes in the local tissue. The experiment was devised to test this.
Methods: The patient drank chilled water while the impedance at 6 stomach-related acupoints was monitored in real time (sample rate of 1 kHz). Any changes in the local tissue at the acupoints ought to be reflected in changes in the impedance.
Results: The impedance at every test acupoint showed a response to the chilled water being ingested. Also, the duodenal pacesetter and the stomach's slow waves were clearly visible in the impedance pattern at all the acupoints.
Conclusion: Hence, many separate details of the stomach's function were reflected at these acupoints. The duodenal features were consistent with the traditional indications for these acupoints, which are noted to be able to treat intestinal conditions. Therefore, the results were consistent with the hypothesis and also provided a possible explanation for how the use of these acupoints is able to treat intestinal conditions.

4Analysis of Korean Patent Information for Air Extraction Cupping Devices

Background: The purpose of this study was to collate inventors' improvements of air extraction cupping technology by reviewing patent applications in Korea. The cost of procurement and use of air extraction cupping devices are covered by health insurance.
Methods: Patents registered in Korea for air extraction cupping devices from August 1992 to January 2018 (registration, expiration, rejection, abandonment, and disclosure) were analyzed to determine technology trends. The Korean patent search engine used was Kipris (www.kipris.or.kr).
Results: Sixty-seven Korean patents for air extraction cupping devices were retrieved. Most patents focused on design of the top valve of the device, the device at the bottom, an internal device, and the disposable cupping glass. In total 17.9% of patents were registered and 64.2% were abolished [either from non-payment of registration fees (93%) or expiration of the patent after 10 years (7%)]. The average registration period was 3.7 years.
Conclusion: The patents retrieved for this study focused on the development of the air exhaust valve, disposable cups, and skin adhesion maintenance technology in cupping devices that use air extraction. The average registration period was 3.7 years, meaning the patents that were not commercialized, expired without additional registration renewals. It is important to acquire a New Excellent Technology certificate for a cupping device to promote commercialization.

5Effects of Galgeungyulpitang on Cellular Production of Melanin and Elastase

Background: This study was designed to investigate the potential effects of Galgeungyulpitang for whitening and elasticity treatment by examining its effect on melanoma cells.
Methods: The effects of Galgeungyulpitang on B16/F10 melanoma cell viability, production of melanin, tyrosinase and elastase, were investigated. Cell viability was measured by colorimetric assay that assesses cell metabolic activity (MTT assay). Melanin was measured by Hosei's method, tyrosinase was measured by Yogi's method and elastase was measured by James's method.
Results: At concentrations higher than 500 μg/mL Galgeungyulpitang, cell viability was significantly reduced (p ≤ 0.05). At concentrations of 500 μg/mL and lower, morphological changes were not observed. The rate of melanin synthesis was significantly reduced to 73.49% ± 2.92% at a concentration of 500 μg/mL Galgeungyulpitang compared with untreated cells (p < 0.05). Extracellular tyrosinase production was not significantly decreased in vitro, however, intracellular tyrosinase production was significantly reduced to 76.06% ± 2.17% when treated with Galgeungyulpitang at a concentration of 500 μg/mL compared with the control (p < 0.05). Elastase Type 1 production was significantly reduced to 74.98% ± 3.24% and 69.62% ± 4.66% at concentrations of 250 and 500 μg/mL Galgeungyulpitang, respectively (p < 0.05). Elastase Type 4 production was significantly reduced to 72.77% ± 3.52% at concentrations of 250 and 500 μg/mL (p < 0.05).
Conclusion: The results in this study showed that Galgeungyulpitang may inhibit melanin and tyrosinase synthesis, and inhibit elastase production, suggesting that Galgeungyulpitang may be potentially beneficial for skin whitening and loss of skin elasticity treatments.

6Illustrations of the Nine Types of Needles based on Huangdi's Internal Classic Ling-shu

Background: The phrase “Nine Needles” refers to the 9 types of acupuncture needles describing their shapes, sizes, and uses in Huangdi's Internal Classic Ling-shu. The aim of this study was to present 3D illustrations of the “Nine Types of Needles” based on Huangdi's Internal classic Ling-shu, taking into consideration the conformation and application of the Nine Needles.
Methods: Sketches of the “Nine Needles” were based on references to those needles cited in Huangdi's Internal Classic Ling-shu, the Great Compendium of Acupuncture and Moxibustion, and the Golden Mirror of Medicine. The computer programs Creo 3.0, Keyshot 5, Adobe Photoshop CS5, and Adobe Illustrator CS5 were used for 3D modelling and visualization.
Results: Based on a review of Huangdi's Internal Classic Ling-shu, illustrations of the Spade needle, Round- Pointed needle, Pressure needle, Sharp-Edged needle, Sword-Shaped needle, Round-Sharp needle, Fine needle, Long needle, and Large needle, 3D models were created. The Spade needle had a sharp, large head, the Round-Pointed needle had an egg-shaped tip, and the Pressure needle had a blunt head like a grain of millet. The Sharp-Edged needle had a sharp blade with a triangular edge for bloodletting. The Sword-Shaped needle resembled a sword. The Round-Sharp needle resembled a horse's tail. The Fine needle and the Long needle had sharp points and thin bodies. The Large needle had a cylindrical shaft and rounded tip.
Conclusion: This study demonstrated that 3D illustrations could be generated for the Nine Needles according to the descriptions and figures provided in the ancient literature.

7A Case of Syringomyelia with Back and Shoulder Pain

The purpose of this study was to describe the treatment of the clinical symptoms of syringomyelia using Korean medicine. A patient with syringomyelia complained of back and shoulder pain, numbness of the upper right limb, headaches and dyspnea. He was treated with herbal medicine, acupuncture, moxibustion, and physical therapy. The Visual analog scale (VAS) was used to score sensory symptoms (pain, numbness) and Modified Borg scale (mBorg) was used to score dyspnea. After the treatment, his symptoms were relieved. The patient's chief complaint was back and shoulder pain which was reduced greatly from a VAS score of 6 between 1-2. The results in this study may contribute to the development of the Korean medicine field of syringomyelia in the future.

8The Effect of Postoperative Korean Traditional Medicine for the of Proximal Humeral Fracture: A Case Report

The purpose of this case study is to examine the effects of Korean traditional medicine in the postoperative treatment of proximal humeral fracture. The patient with postoperative pain following proximal humeral fracture surgery was treated with Korean traditional medicine including acupuncture cupping therapy and herbal medicine in hospital for 67 days. The effects of the treatment were evaluated using the Numerical Rate Scale (NRS), Range Of Motion (ROM), and Shoulder Pain and Disability Index (SPADI). As a result of treatment, the NRS score was reduced from 7 to 3, the ROM was improved (on average by 21%), SPADI was reduced from 93 to 25.9. This study proposes that Korean traditional medicine may be effective in postoperative treatment for proximal humeral fracture.

9Hemifacial Spasm Treated by Thread-embedding Therapy

The aim of this study was to investigate the efficacy of treatment with thread-embedding therapy for 24 patients with hemifacial spasm (HFS). The muscle spasm of these patients was treated with thread-embedding therapy. Patients with nuchal pain were treated with tendino-musculature acupuncture in the sternocleidomastoid, splenius, and trapezius muscles. We evaluated the treatment effect using the Scott's scale, where 20, 3, 1, and 0 patients presented Scott's grade 0, grade 1, grade 2, and grade 3, respectively. The grade of the spasm intensity decreased noticeably after treatment. The results revealed that the Scott's grade changed to 0 in 83.3% of HFS patients, and 91.7% patients felt satisfied with thread-embedding therapy. These findings suggested that thread-embedding therapy was effective and can be used widely for HFS.
